The role:
We are excited to offer an excellent opportunity within NHS Greater Glasgow and Clyde for a Rehabilitation Physiotherapist to join our innovative Rapid Assessment Link service. You will work as part of a highly skilled multidisciplinary team, delivering specialist rehabilitation to patients with complex needs. Key responsibilities:
- Deliver specialist physiotherapy assessment, treatment and evaluation for patients with complex rehabilitation needs

Work autonomously, managing a defined caseload while contributing to multidisciplinary care planning and coordination

Support early discharge from hospital and prevent avoidable admissions through effective community intervention
-
Act as a key link between acute and community services, ensuring seamless patient care pathways
-
Provide expert advice, education and support to patients, carers and multidisciplinary colleagues
-
Contribute to service development, clinical audit and quality improvement initiatives within the team

Supervise and support rehabilitation assistants and contribute to staff development and training
Knowledge, training, qualifications and/or experience required to do the job:
- Degree or MSc in Physiotherapy with current HCPC registration

Significant post-graduate experience, including time at a senior level within relevant clinical areas

Proven clinical expertise in rehabilitation, including care of older adults, neurology, orthopaedics and respiratory conditions

Ability to work independently and demonstrate advanced clinical reasoning in complex patient care

Strong communication, interpersonal and organisational skills with the ability to work across multidisciplinary teams

Evidence of ongoing professional development and application of learning to clinical practice

Full driving licence and willingness to work across community settings
