We are seeking a motivated and organised Lead Care Navigator to join our team on a part-time basis. This is a key non-clinical role that will work alongside the current Lead Care Navigator to provide leadership and support to the Care Navigation Team, ensuring effective cover and service delivery across Monday to Friday.
The successful candidate will play a vital role in helping patients navigate the healthcare system by coordinating care, managing patient information, and ensuring individuals receive the appropriate support, resources, and guidance.

Main duties of the job
Key Responsibilities:
- Support and oversee the daily operation of the Care Navigation Team.
- Provide guidance, leadership, and support to team members.
- Coordinate patient care and signpost patients to appropriate services.
- Maintain accurate and confidential patient records.
- Communicate effectively with patients, healthcare professionals, and external agencies.
- Ensure a high standard of patient service and administrative support.

- Previous administrative experience, ideally within a healthcare or customer service environment.
