The post holder will lead and deliver clinical pharmacy services to patients within the HIV AND Sexual Health service, ensuring safe, effective and person-centred pharmaceutical care. This includes:

Leading delivery of pharmacist-led medication reviews, including annual and complex interaction reviews

Supporting prescribing, monitoring and optimisation of antiretroviral therapy

Providing expert advice on complex regimens, adherence, and side effect management

Supporting homecare and coordination of medicines supply

Contributing to MDT working within Sexual Health and HIV services

Supporting service development to improve prescribing efficiency, safety and patient experience

Releasing medical and nursing capacity through advanced pharmacy practice
Key Responsibilities
- Provide clinical pharmacy leadership within the HIV service
- Deliver advanced pharmaceutical care to patients with complex treatment regimens
- Support prescribing, results monitoring and timely medication adjustments
- Lead medicines optimisation initiatives to improve outcomes and efficiency
- Contribute to governance, audit and quality improvement activities
- Support development of systems and processes to improve prescribing workflows
- Work collaboratively across multidisciplinary teams and with pharmacy colleagues
- Contribute to service development and workforce innovation within Sexual Health services

As from 1/4/26, the Agenda for Change full-time working week will be reducing from 37 to 36 hours per week. Part time hours will be reduced pro-rata. However, there will be a corresponding increase in the hourly rate so that pay will not be affected.
