An exciting opportunity has arisen for the position of Advanced Clinical Practitioner in Cardiology, based across Morecombe Bay. The successful applicant will either be a qualified ACP or be required to complete the ACP course based at a local University at masters level.
The post holder will be required to travel between all three hospital sites and community sites within UHMB including at Kendal, Lancaster and Barrow-in-Furness.
The role blends direct, higher level clinical care with education, management activities with a focus on Governance and as such the post holder is expected to build working relationships across the local health community. The ability to initiate, sustain and evaluate change is essential.
The Cardiology ACP will work closely with the existing Heart Failure team and a team of multi-disciplinary staff within the CCDE and MECCS Directorates, including Consultant Cardiologists, Specialist Cardiology Nurse, and Clinical Investigation Technicians.
The successful candidate will be an expert nurse within cardiology; leading and developing practice in order to promote and provide high quality service throughout the patient pathway for patients and their families.
Provide and facilitate the delivery of highly specialised care for patients with heart failure. To include clinics and home visits.
Be responsible for providing expert practice through leadership, consultation, clinical advice, teaching and support for staff. The post holder will work with clinical colleagues to provide information, advice and support to patients with complex heart failure.
Service development will be an integral part of the post going forward.
We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
