Enhancement Additional Payments: £0.86 - Unsocial Hours, 1.5 x hourly rate - Bank Holiday

About the service
Stewart Gardens is part of our Devon Supported Housing service, providing housing-related support to people leaving hospital or residential settings who have low to medium support needs. Using a recovery-focused approach, we empower people to build confidence, develop independence, and achieve their personal goals.
What you'll be doing
As a Mental Health Support Worker, you'll work closely with the people we support, helping them take positive steps towards greater independence and wellbeing. Your role will include:
- Providing person-centred support tailored to individual goals and recovery plans.
- Building positive, professional relationships based on trust, respect and encouragement.
- Supporting people to access local services, community opportunities, education, employment and social activities.
- Undertaking needs assessments, support planning and risk management activities.
- Working collaboratively with healthcare professionals, external agencies and community partners.
- Maintaining accurate records and contributing to service quality and outcomes monitoring.
- Promoting wellbeing, independence and self-management skills.
- The role may also involve supporting with personal care where required
Essential experience, skills required for this role:
- I have experience of providing support to people with a mental illness in a social care setting.
- I have transferable life/work experience to apply to this role.
- I have experience of making a comprehensive assessment of an individual’s situation and needs; to develop, implement and review risk management plans and devise support plans and activities to meet these needs.
- Ability to work with digital systems (Microsoft applications, email, internet)
- I am a good listener, empathetic, understanding, kind, patient and willing to learn
- I can demonstrate behaviours consistent with the values of the Charity.
