An exciting opportunity has arisen for a Senior Community Practitioner to join the East Integrated Community Mental Health Team based in Northallerton.
The East Integrated Community Mental Health Team provides mental health services and delivers care to adults across Hambleton area.
As a member of the team, you will work alongside all members of the multi-disciplinary team which includes Registered Nurses, Social Workers, Psychologists and Psychology Assistants, Nurse associate, Occupation Therapist, Consultant Psychiatrists and medics, Team admin. With access to and support from specialist services available in the trust, and excellent working relationships with local partner organisations.

As a key worker the successful postholder will be expected to hold a case load of patients, and be able to deliver care to meet the identified patients needs autonomously and with the support from the MDT.
You will be required to participate in the duty worker rota and support the patient journey from referral to discharge.
In addition to this, as a senior community practitioner, you will be expected to provide support to junior members of the team and to provide an excellent learning environment for students who are placed with the team.
Candidates must have registration with the relevant governing body. Nursing candidates must have 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework at the time of applying.
