About the Role
Location: South West England/Plymouth
Overview
We are looking for a skilled, compassionate and experienced Brain Injury Case Manager Associate to join our specialist team. You will support adults living with acquired and traumatic brain injury in the community, delivering high-quality, client-centred case management that promotes autonomy, rehabilitation, safety and meaningful participation in everyday life.
This role is ideal for a confident practitioner with at least five years’ experience in brain injury, neurorehabilitation or complex care, who holds current BABICM membership. Working collaboratively within a multidisciplinary framework, you will coordinate, monitor and optimise care and rehabilitation pathways across the South West.
Key Responsibilities
Holistic Assessment & Planning
Conduct comprehensive assessments of clients’ physical, cognitive, emotional, behavioural, social and environmental needs. Develop and implement tailored case management plans aligned with rehabilitation goals and best practice.
Care Coordination & Oversight
Coordinate complex care packages, ensuring safe, effective and person-centred delivery. Liaise with care providers, therapy teams, medical professionals and statutory services to maintain continuity and quality.
Risk Management
Review and update risk assessments and care plans regularly. Identify emerging risks and implement proactive strategies to maintain safety and wellbeing.
Rehabilitation Support
Work closely with treating therapists to support rehabilitation programmes, monitor progress and adjust interventions as needed.
Client Advocacy
Promote clients’ autonomy, rights and choices. Ensure their voice is central to all decision-making and that they are supported to engage meaningfully in their community.
Professional Liaison
Build strong working relationships with legal teams, Deputies, NHS services, community providers and other stakeholders to ensure seamless service provision.
Monitoring & Reporting
Maintain accurate and timely documentation. Prepare professional reports, updates and recommendations for legal teams, funders and multidisciplinary partners.
Service Quality & Compliance
Uphold BABICM standards, regulatory requirements and organisational policies. Engage in reflective practice, supervision and ongoing CPD.
Essential Criteria
· Minimum 5 years’ experience working with individuals with acquired or traumatic brain injury, neurological conditions or complex care needs.
· Current BABICM membership, Standard or Enhanced.
· Strong understanding of neurorehabilitation, cognitive and behavioural presentations, and community-based recovery.
· Experience coordinating multidisciplinary input and managing complex care packages.
· Excellent communication skills, with the ability to work collaboratively with families, professionals and legal representatives.
· Ability to work autonomously, prioritise effectively and maintain professional boundaries.
· Commitment to ethical, client-led practice and continuous professional development.
· Full UK driving licence and ability to travel across the South West.
Desirable Criteria
Experience working within medico-legal or litigation contexts.
Knowledge of the Mental Capacity Act, safeguarding frameworks and community health pathways.

Pay: Associate Role – Percentage sliding scale relevant to experience.
