About the Role

Intuitive Thinking Skills is seeking a dedicated and resilient Housing First Caseload Lead to support our team in Denbighshire. This role focuses on working with individuals experiencing chronic homelessness, helping them secure and sustain permanent housing through a Housing First approach.

You will provide informed, person-centred support to individuals with complex needs, including mental health challenges, substance use, offending history, and social exclusion. This is a rewarding opportunity to make a tangible difference in people’s lives by supporting long-term stability and independence.

Key Responsibilities

1. Caseload Management

Support a team of staff with a defined caseload of clients who have multiple social needs
Confidently challenge attitudes and behaviours where appropriate
Build strong, trusting relationships with both staff & clients
Conduct regular home and community visits to support tenancy sustainment and encourage positive change

2. Housing Support

Support clients in accessing and maintaining suitable, permanent accommodation
Help individuals understand their tenancy rights and responsibilities

· Liaise with Social Landlords, Private Landlords, Homeless prevention team, Homeless private rented team to assist with the citizen living in accommodation of their choice, including practical help with furniture, benefits advice, accompany visits and any liaison with landlords.

3. Person-Centred Support Planning

Co-produce tailored support plans based on individual strengths and goals
Enable access to key services including healthcare, mental health, substance misuse, and employment support
Promote independence, choice, and control

4. Crisis Intervention & Safeguarding

Deliver outreach support to rough sleepers across the county
Respond effectively to crisis situations, including risk of eviction or declining wellbeing
Identify and manage risks, including safeguarding concerns, in line with policies
Work collaboratively with partner agencies to ensure client safety
Manage the ROC Hub each Wednesday, supporting Sanctuary Trust staff and encouraging engagement

5. Multi-Agency Working

Build and maintain strong partnerships with statutory and voluntary organisations
Attend case conferences, reviews, and multi-disciplinary meetings
Advocate on behalf of clients to ensure access to appropriate services

6. Record Keeping & Reporting

Maintain accurate and up-to-date records using Open Housing
Monitor outcomes and contribute to reporting requirements
Ensure compliance with data protection and organisational policies

About You

We’re looking for someone who is:

Passionate about supporting individuals experiencing homelessness
Confident in challenging attitude and behaviour
Experienced in working with people with multiple needs
Skilled in building relationships and engaging hard-to-reach individuals
Resilient, adaptable, and confident in managing challenging situations
Relevant lived experience is desirable but not essential
A strong communicator with the ability to work both independently and as part of a team

Pay: £33,000.00-£35,000.00 per year
