To deliver a comprehensive, high quality evidenced based Cardiac Rehabilitation service in collaboration with the Multi-disciplinary Teams across Primary, Community and Secondary Care along integrated pathways of care in Powys .
Actively contribute to the planning and development of the Cardiac and Heart Failure services across Powys within the Community Cardiology Service .
This role will include delivery of exercise based cardiac rehabilitation in community settings across North and Mid Powys. Including patient assessment, medication review, appropriate follow up and evaluation of outcome measures .
Work collaboratively with other professions e.g., Physiologists, GPs, Consultants, Nurses and AHPs to provide expert patient centred care.
Lead aspects of service evaluation with an emphasis on value-based care and complex audit within the service .
As part of the Community Cardiology Service provide training and development to Powys Teaching Health Board Staff and partners on coronary heart disease and heart failure management, treatment, and care .
Work within and adhere to the code of conduct for professional body; standards and guidance at all times , ensuring that issues of concern are escalated appropriately. Act at all times as a role model, presenting a professional approach and ensuring the standards for professionalism are set and displayed within the team .
Responsible and accountable for an identified case load, proactively promoting health enhancing activities and behaviours, working with the patient/carer and the MDT team to reduce unplanned hospital admissions and co-ordinate care across all settings of the care pathway .
Provide skilled assessment, treatment, and advice to adult patients in cardiac rehabilitation. Undertake the lead role in the assessment, diagnosis, and formulation of cardiac rehabilitation treatment plans.
P romote and ensure through ongoing review that all patients are treated in line with the principle of dignity and respect and that this is embedded within the team .
Involve patients, relatives, and carers in the planning of treatment programmes including overcoming barriers to communication, gaining consent for assessment and interventions, and providing advice and education .
Support patients following a virtual/digital cardiac rehabilitation programme and to support patients following a home-based programme with use of the Heart Manual.

Provide specialist, comprehensive assessment and management of patients following myocardial infarction (MI), percutaneous coronary intervention (PCI), coronary artery bypass grafting (CABG), valve repair/replacement, left ventricular systolic dysfunction (LVSD), and other cardiac conditions.
Utilise advanced specialist knowledge and clinical skills to support the treatment, rehabilitation, and ongoing management of cardiac patients.
Lead the delivery of community-based cardiac rehabilitation services across North and Mid Powys, supported by a Band 4 Cardiac Assistant Practitioner and National Exercise Referral Scheme (NERS) Exercise Instructor. This includes patient education, exercise assessment/testing, programme delivery, and evaluation of outcome measures.
Optimise patients’ cardiovascular health through independent prescribing (Non-Medical Prescribing qualification) or a commitment to undertake the qualification within 12 months of appointment, with the aim of reducing cardiovascular risk factors and symptom burden.
Participate in the National Audit of Cardiac Rehabilitation (NACR) and All Wales Cardiac Rehabilitation meetings, contributing to service development, quality improvement, and audit activities.
Work as an integral member of the wider cardiology multidisciplinary team (MDT), collaborating closely with colleagues to support symptom management, patient education, self-management, and personalised care planning.
Demonstrate excellent communication and interpersonal skills, with the ability to engage effectively with patients, carers, colleagues, and professionals across health and social care settings.
Be able to travel independently and work across a range of clinical and community settings to meet service requirements.
Demonstrate flexibility and adaptability in working practices to meet the evolving needs of the service and patient population.
