ADVANCED CLINICAL PRACTITIONER
24 hours per week over 3 days (Monday, Tuesday and Thursday)
Salary: £56,169 - £63,222 (Pay award pending) Negotiable dependent upon qualifications and experience
We are looking for an enthusiastic and experienced Advanced Clinical Practitioner with an independent prescribing qualification to join our busy and friendly GP practice. You will perform as an autonomous practitioner working independently and as a part of multidisciplinary team to assess, diagnose and treat patients with a range of acute, non-acute and chronic medical conditions. You will have significant practical experience, ideally within primary care.
You will work alongside the GPs providing minor illness clinics. You will possess or be actively working towards an Advanced Clinical Practitioner qualification.

JOB PURPOSE
· An experienced clinician, able to work independently to deliver a high standard of patient care using advanced autonomous clinical skills and a broad, in-depth theoretical knowledge base.
· To manage a clinical caseload dealing with the presenting patient’s undifferentiated and undiagnosed problems within the primary care setting.
· Providing care from initial history taking to clinical assessment, diagnosis, treatment/referral and evaluation of care.

· Providing a key clinical input into maximising QOF and Enhanced Services’ income for the practice
· The delivery of cost-effective, high quality patient care, including prescribing, in line with local and national guidelines
KNOWLEDGE, SKILLS AND RESPONSIBILITIES
Essential
- Registered clinician who has undertaken an accredited advanced clinical practice programme, ideally at Master’s level.
- Minimum of 3 years’ experience of working in a primary care setting
- Minimum of 1 year working as an Advanced Clinical Practitioner (ACP) with proven achievement within this role
- Self-directed, motivated practitioner
- Team player; positive attitude; problem solving approach
- Independent prescriber
- Able to organise and manage own workload, ability to work under pressure and to effectively prioritise and ensure good time management
- Confident of own knowledge base, awareness of learning needs and seeks guidance when appropriate
- Able to critically review and communicate clinical information both to patients and professionals
- Excellent communication and interpersonal skills
- Sound IT skills; willingness to embrace technological development in the role
- Demonstrates flexibility regarding hours and responsibilities commensurate with a senior role
- Evidence of continuing personal and professional development
- Awareness of clinical governance issues in primary care
Desirable
- 3 years’ experience as an ACP
- Practical experience of SystmOne clinical system
- Mental health experience and/or training
- Knowledge and practical experience of QOF, local care pathways, local initiatives and enhanced services
- Understanding of Primary Care developments and future initiatives within the NHS
- Involvement in clinical audit and peer review processes
KEY RESULT AREAS
Clinical
- Independently prioritise, treat and safely discharge or refer patients attending with undifferentiated and undiagnosed urgent or chronic primary health care problems
- Provide safe, evidence-based, cost-effective, individualised patient care
- Undertake telephone and face to face consultations as required/appropriate
- Work flexibly alongside the medical team to meet demand on the day
- Make professionally autonomous decisions for which the post-holder is accountable
- Instigate diagnostic tests or investigations as appropriate and interpret findings/reports. Take an active role in the processing of pathology results and correspondence, taking action as directed by secondary care/MDTs, etc.
- To prescribe safe, effective and appropriate medication as defined by current legislative framework
- Assess, manage and refer patients presenting with mental health needs in accordance with NICE guidelines for mental health and wellbeing
- Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
- Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based interventions into a management plan
- Liaise with other agencies and health professionals to ensure collaboration across the primary/secondary care interface to meet the needs of the patient
- Contribute to the practice achieving its quality targets and actively participate in the ongoing review, improvement and development of patient care and service delivery
- Communicate effectively with patients and carers; ensuring patients are fully informed, understand and consent to treatment. Communicate sensitively with and support patients who are receiving ‘bad news.’
- Contribute to the development, implementation, review and evaluation of clinical policies and protocols
Professional Role
- Maintain professional registration
- Promote evidence-based practice through the use of the latest research-based guidelines and the development of practice-based research
- Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of audit and peer review
- Maintain accurate, contemporaneous records, in line with good practice
- Work collaboratively with colleagues within and external to the practice
- Work closely with the doctors and managers in the setting up and/or review of practice systems for monitoring/measuring performance against clinical governance and quality targets
- Participate in teaching and mentoring other health professionals and members of the primary health care team
- Ensure clinical supervision needs are met
- Ensure adherence to all Practice policies and procedures
- To attend and participate in practice meetings including clinical, medicines management and educational (Quest) meetings. To undertake training as required including mandatory and core skills training
- Participate in continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained
