Are you a registered Mental Health Nurse, Occupational Therapist, or Social Worker looking for a role that offers flexibility around your existing commitments? We have an exciting opportunity for a Community Mental Health Practitioner to join our Crisis Home Resolution Team Access Pathway on a part-time basis, on fixed term contract until March 2028 .
We currently have 25.5 hours per week available, ideally worked Monday to Friday between 4:00pm and 9:00pm. We recognise that flexibility is important, so we are happy to consider job share arrangements and can discuss working patterns or specific days that suit both you and the service.
The MH Practitioners have a crucial role in processing, assessing and determine the mental health and social care needs of a person referred to determine their ongoing needs and facilitation of service provision. In addition, the role will require excellent risk assessment and management skills in relation to and positive risk taking and decision making to enable safe and effective responses for an identified client group, largely by telephone/assessment.
This role offers the opportunity to work predominantly from home if you prefer , providing flexibility while remaining connected to a supportive and experienced team. However, you will be required to attend either Hall Court, Telford , or The Redwoods Centre, Shrewsbury , on occasion for meetings, training, supervision, or service requirements.
Leading on the assessment of people referred to the Access Team.
Development and maintenance of effective communications within the area of work and promotion of positive working relationships with pathway colleagues, departments and other agencies.
Managing time effectively to ensure the best use of staff and resources, informing senior members of the team/pathway when issues impacting on service delivery become apparent.
