Join Our Team as a Locality Flow Broker
Job Title: Locality Flow Broker (3 posts, West Cornwall, Central Cornwall, North and East Cornwall)
Location: Hayle, Cornwall (with free parking!)
Salary: £26,000 per annum
Hours: 37 per week | Mon–Thu 9am–5pm, Fri 9am–4:30pm. An earlier start is also an option.
Contract: Permanent subject to continued funding

Role Purpose
To act as a key point of contact within a locality team, within our Humans Cornwall Hospital Discharge Service, ensuring a safe, person-centred, and timely discharge pathway for people leaving hospital. The postholder will represent Humans Cornwall at Multi-Disciplinary Team meetings, collaborate with NHS, private and VCSE partners, manage referrals into the service escalating and de-escalating clients between partners from and into Humans, and advocate for the most appropriate support for each referral, ensuring VCSE engagement is considered from the earliest possible stage, aligned with our contracted targets.
Key Responsibilities:
MDT & Partnership Working
· Attend MDT meetings for the respective locality team with NHS teams, partner organisations and Adult Social Care.
· Act as the main Humans representative for your respective locality, supporting communication and information sharing between partners.
· Contribute to discussions regarding client needs, support options, and service availability using a person-centred and strengths-based approach.
· Support the coordination of appropriate referrals and timely access to support within the locality.
· Build positive working relationships with partner organisations to support joined up and responsive service delivery.
Referral Management & Triage
· Receive, review, and triage referrals into your respective locality team in line with pathway criteria and agreed response times.
· Prioritise referrals based on urgency, discharge timelines, and identified risks or barriers to safe discharge.
· Ensure referrals are accurately recorded on the client management system and allocated appropriately to the team for commencement of support.
· Maintain accurate and timely records of referrals, discussions, and client updates.
· Monitor client flow and active support journeys within the locality, escalating concerns, delays, or changes in need to operational managers where appropriate.
Client Coordination
· Support timely engagement with Humans services, including coordination of pre-discharge interventions within agreed timescales.
· Maintain communication with partner agencies to support smooth transitions between hospital, community, and escalating/de-escalating clients.
· Liaise with Brokers to support continuity and progression through support pathways.
· Highlight delays, barriers, or risks impacting discharge or support arrangements and escalate where appropriate.
Operational Support, Performance & Quality
· Support the monitoring of referral activity, service demand, and client flow within the locality.
· Contribute to the collection of performance information and reporting requirements linked to service outcomes and contractual KPIs.
· Ensure support timelines are met, and client journeys are progressed appropriately through the service.
· Work collaboratively with operational managers and wider teams to support safe, responsive, and effective service delivery.
Essential Skills and Attributes:
· Ability to communicate confidently and professionally within multi-agency environments.
· Confidence to appropriately raise concerns, ask questions, and challenge decisions constructively where required.
· Understanding of hospital discharge pathways, reablement, and community-based support services desirable.
· Ability to manage and prioritise referrals in a fast-paced environment while maintaining accuracy and professionalism.
· Competent in record-keeping, data entry, and the use of digital systems.
· Calm, organised, and solutions-focused when managing competing priorities and service pressures.
· Compassionate, person-centred, and approachable, with strong listening and communication skills.
· Able to build positive working relationships with clients, carers, colleagues, and partner agencies.
· Able to work independently within agreed processes while recognising when to escalate concerns to managers.
