Horizon Care and Education have an exciting opportunity to join as a Youth Support Worker must be willing to cover the areas of Staffordshire, Stoke & Telford.

As a Youth Support Worker you will be helping young people transition to independent living, duties include:

Provide emotional and practical support, including out-of-hours on-call.
Develop and review support plans, ensuring safeguarding and risk management.
Help young people access education, training, and employment.
Manage properties and liaise with other professionals.
Support our young people to develop independent living skills to prepare them to live fully independently.

What we’re looking for from you:

Experience working with vulnerable young people, ideally care leavers.
Strong safeguarding knowledge and assessment skills.
Ability to be resilient and empathetic when working with our young people.
Excellent communication and problem-solving abilities.
Flexibility to be able to work shifts that include evenings, weekends and sleep ins.
A driving license and access to a vehicle.
