Job Summary
JOB TITLE: Health Care Assistant
Reports to Senior Health Care Assistant and Nurse Team Manager and ultimately the Partners of PMP.
Practice Manager - administratively
Role To work as part of the clinical team supporting the doctors and nursing team, delivering care within the boundaries of the role.
Summary
Delivery of HCA services, focusing on supporting patients to be healthy, monitoring of long-term conditions, health promotion and screening activities.
- Effective use of practice IT systems to document and manage care in line with our data protection and IT policies. .
- To participate in all training identified as necessary to develop skills necessary for the role.
MAIN DUTIES AND RESPONSIBILITIES
Work collaboratively with the practice teams to undertake all duties in line with the training and instruction provided in a professional manner whilst maintaining patient comfort and dignity at all times.
Clinical Practice:
1. Demonstrate good understanding of principles of consent and ensure valid consent is obtained prior to undertaking nursing and care procedures.
2. Undertake venepuncture, urinalysis and the taking and preparing of pathology samples using ICE, including careful use of the centrifuge if required and recording results on the appropriate template.
3. Assist the clinical team to promote health and well-being, and prevent adverse effects on health including the provision of healthy lifestyle advice and promotion regarding diet/exercise/smoking and alcohol. Including recording accurate observations (weight, height, blood pressure, pulse rate, temperature etc) in accordance with requirements.
4. Act as chaperone and/or assist patients to dress and undress for procedures if required.
5. Monitor long term conditions in line with the HCA practice policies and ensure correct recall procedures and diary entries are accurate and complete.
6. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care and BLS.
7. Fit and remove 24-hour BP monitors and upload results.
8. Perform ECG tests seeking immediate advice if necessary and informing requesting clinicians of results.
9. Administer influenza, pneumococcal, shingles vaccines and B12 injections under indirect supervision of regulated nurse and in accordance with relevant PSD’s. Promote health and prevent ill health by understanding the evidence base for immunisation, vaccination and herd immunity.
10. Undertake near patient testing for INR and warfarin management, using INR STAR software and following practice protocols for informing patients and recording management.
11. Develop a working knowledge of other providers’ resources and referral systems to ensure individual’s needs are met, within parameters of practice. Familiarise yourself with the ARRS roles within the PCN to enable appropriate signposting and referral.
12. Maintain effective and sensitive communication at all times promoting collaborative working across multidisciplinary teams, promoting the principles of equality and diversity and acting as the patients advocate if necessary.
Training and Development
13. Be proactive in seeking opportunities to develop own knowledge and skills, achieving competencies. Seeks support / guidance in timely manner if any difficulties are encountered.
Administration:
14. Assist in the monitoring and maintenance of adequate stock levels in all clinic rooms and the dressing cupboard, including monitoring expiry dates, ensuring your work space is kept clean, tidy and adequately stocked for the next user.
15. Take an active role in the management of the cold chain ensuring daily recording of fridge temperatures, timely and appropriate storage of all vaccines, reporting any excursions outside of 2-8˚C immediately to the senior nurse on duty. Understand and apply the principles of the cold chain and partake in cold chain audit if necessary.
16. Facilitate the collection and redistribution of equipment for annual calibration and PAT testing.
17. Produce accurate, complete, and contemporaneous patient records and all documentation associated with patient care using agreed Read Codes and templates. Undertake annual audit of records to ensure record keeping is of a high standard.
Infection Control
18. Ensure clinical equipment is regularly cleaned and prepared in accordance with relevant protocols and recorded appropriately.
19. Participate in infection control audits and training as required.
20. Understand you have a personal responsibility to reduce infection spread; ensure you are vaccinated against all recommended infections including hepatitis B, Covid-19 and influenza.
21. Ensure the correct collection of clinical waste, including sharps, ensuring appropriate storage prior to disposal.
22. Ensure familiarity with the current infection control policies paying particular attention to the correct hand hygiene procedures and use of personal protective equipment.
Professional Conduct and Practice Development
23. Work at all times within the limits of your own knowledge and competence and within the established polices and standards of the Practice, referring to senior clinicians for support, guidance and advice as necessary.
24. Use initiative to identify to Senior HCA areas where improvements in patient care or team working will actively enhance the aims and purpose of the Practice. Work towards Quality Improvement Exercises independently or with others as part of annual appraisal outcomes.
25. Contribute to audit and other data quality/service improvement interventions, including participation in shared learning across the practice and review and reporting of significant events.
26. Actively support other members of the HCA or nursing team to ensure fair distribution of work load and strong team support.
27. Accept delegation from other clinicians, prioritise own workload and ensure effective time management strategies are embedded in own practice.
28. Maintain awareness of own learning needs, actively participating in annual appraisal and taking personal responsibility to complete all mandatory and statutory training requirements
29. Act as a positive role model for students and visiting clinicians, supporting learning as necessary and promoting our role as an Advanced Training Hub.
30. Maintain a safe and therapeutic environment for yourself and others in accordance with the Management of Health and Safety at Work and COSSH Regulations.
31. Promote a professional approach at all times by attire and attitude to patients and staff alike, both in and out of the Practice
32. Attend and participate in staff and Practice meetings and in-house clinical training sessions
33. Work at satellite site of Thornton Dale and participate in community health care if required.
Confidentiality and Data Protection
34. Maintain confidentiality and manage information sensitively in line with practice policies.
35. Undertake annual Information Governance training
36. Understand the importance of the GDPR (2018) regulations, Data Protection Act 1998 and the Human Rights Act 1998 and know that disclosures of confidential information or data of a personal nature can result in prosecution in addition to any action that the Practice deems necessary.
37. Keep safe your smart card, keep the door to your room closed when not using it and switch off computer screens to avoid unintentional access.
