24-7 Homecare Services Ltd is recruiting an experienced Care Coordinator / Care Scheduler / Care Supervisor to join our growing domiciliary care team in Doncaster.

Key Responsibilities
As a Care Coordinator, you will:
- Create and manage staff rotas and schedules (considering travel, availability, and continuity of care)
- Allocate care calls and manage new care packages / referrals
- Conduct client assessments, reviews, and care planning
- Develop and maintain care plans, risk assessments, and documentation
- Monitor care quality through spot checks, audits, and supervision
- Liaise with clients, families, and healthcare professionals (GPs, district nurses, social services)
- Respond to changes, emergencies, and staff shortages
- Support recruitment, induction, and training of care staff
- Ensure compliance with CQC standards, safeguarding, and health & safety regulations
- Maintain accurate records using care management systems
- Participate in on-call duties (rota basis)
- Contribute to service improvements and team development

Requirements
Essential:
- Experience in domiciliary care, home care, or community care
- Previous experience as a Care Coordinator, Care Scheduler, Senior Care Assistant, or Supervisor
- Strong organisational and time management skills
- Excellent communication skills (written and verbal)
- Good IT skills (care systems, databases, Microsoft Office)
- Full UK driving licence and access to a vehicle
- Understanding of care planning, risk assessments, and safeguarding
- Ability to prioritise and adapt in a fast-paced environment
Desirable:
- NVQ Level 3 (or above) in Health & Social Care
- Experience using rota / scheduling software
- Knowledge of local health and social care systems
- Previous care coordination or management experience
