About Us
Merton Lodge Surgery is a friendly, forward-thinking rural GP practice serving approximately 7,000 patients in the historic market town of Alford, Lincolnshire.
We are a multidisciplinary team comprising GPs, Advanced Nurse Practitioners, Practice Nurses, Nursing Associates, Clinical Pharmacists, Paramedics, Social Prescribers, Care Coordinators, and an experienced administrative team. We pride ourselves on providing high-quality patient care whilst maintaining a supportive and collaborative working environment.
The Role
We are seeking a qualified, experienced and motivated Practice Nurse or Nursing Associate to join our team on a fixed-term basis to provide maternity cover.
This role is suited to a nurse who is already experienced in primary care and is confident managing patients with long-term conditions and chronic disease. The successful candidate will play an important role in supporting the delivery of high-quality patient care and preventative healthcare services.
Main Duties
The successful candidate will be expected to undertake a range of nursing duties, including:
- Long-term condition reviews and chronic disease management.
- Management of patients with diabetes, asthma, COPD, hypertension, and cardiovascular disease.
- Health promotion and preventative care.
- Vaccinations and immunisations.
- Supporting QOF achievement and population health initiatives.
- Working collaboratively within the wider multidisciplinary team.
- Maintaining accurate and contemporaneous clinical records.
Nurse applicants may also undertake:
- Cervical cytology - not essential for this post.
- Contraceptive services (where appropriately trained).
- Additional enhanced nursing services.
Essential Requirements of Nurse Applicants
- Registered Nurse or Nurse Associate with current NMC registration.
- Previous experience working within General Practice or Primary Care.
- Fully competent in managing long-term conditions and chronic disease reviews.
- Experience managing patients with diabetes, asthma, COPD, hypertension, and cardiovascular disease.
- Competent in vaccinations and immunisations.
- Excellent communication and organisational skills.
Nursing Associate Applicants
- Registered Nursing Associate with current NMC registration.
- Previous experience working within General Practice or Primary Care.
- Experience supporting long-term condition management within a primary care setting.
- Competent in undertaking chronic disease reviews within scope of practice.
- Experience in vaccinations, phlebotomy, and health promotion.
- Excellent communication and organisational skills.
Desirable
- Experience using SystmOne.
- Additional chronic disease management qualifications.
- Experience supporting QOF delivery and quality improvement initiatives.
