As a Peripatetic Care and Support Manager - Assessment and Funding you’ll be a key part of Hft Gloucestershire's Transformation Project to reduce the current financial deficit. Your primary purpose is to maximise support and outcomes for the people we support by ensuring care packages and funding arrangements and support plans accurately reflect assessed need and legal entitlements.
You’ll work closely with your colleagues in the senior leadership team within care and support operations. You will also be required to work collaboratively with colleagues within operations and finance as well as other business support functions.
What You’ll Be Doing
You’re passionate about enabling learning disabled adults to live life their way. You will:

Lead on a project relating to Care Act assessments and funding alignment

Work alongside Regional Service Managers and Service Managers to review support hours against assessed needs, indentify under-funded arrangements, highlight risks and unmet need, develop evidence-based funding cases, prepare submissions for funding panels

Provide guidance to operational staff on the Care Act 2014 including eligibility citeria, outcomes-based assessments and review requirements

Support Service Managers with preparing and attending reviews

Support managers with challenging proposed reductions, representing the case for Hft
-
Support escalation processes to challenge decision making

Work alongside managers to create robust evidence packs using health needs information, incident data, behaviour support records, risk assessments and staffing requirements

Offer guidance to managers on writing effective review reports, presenting evidence and managing funding disputes

Keep records of all successful funding challenges and associated income

Provide up to date reports to the senior leadership team on progress made

Create templates and consistent processes to support this work across Hft
Who You Are
You’re passionate about enabling learning disabled adults to live life their way. You bring:

Experience as a senior social care leader at regional level and used to managing multi-site services
- Extensive operational, transformation and commissioning experience across CQC regulated services
-
Ability to apply the key principles of the Care Act 2014, the Care Act Assessment process involving local authority funders and providers

Knowledge of the Mental Capacity Act 2005 and Deprivation of Liberty Safeguards

Knowledge of funding for supporting adults with learning disabilities, autism, mental health and complex behaviours
-
Skills in assessing and reviewing support needs in line with achieving safe and person-centred outcomes

Experience in working with commissioners, local authorities and multidisciplinary teams

Ability to support and coach operational managers

A valid driving licence and access to your own transport.
