Full job description

About the Role:
Compass House Medical Centres are seeking a compassionate and skilled Carer Support Worker to provide assessments, support, and guidance to unpaid carers within our community. This role offers the opportunity to make a real difference to carers’ health and wellbeing by identifying needs, providing support, and connecting carers with the right resources and services.

Key Responsibilities:

Work alongside GP practice teams as a point of expertise to support unpaid carers.
Conduct Health and Wellbeing Checks and assessments with carers in the practice, at home, or remotely via phone, Teams, or Attend Anywhere.
Use a strengths-based, trauma-informed approach to identify carers’ needs, outcomes, and Care Act eligibility.
Support carers to access appropriate services and community resources, including emotional and practical support.
Refer carers to specialist support services when necessary (e.g., Mental Health, Dementia, Young Adult, Learning Disability, Substance Misuse, or Children’s Services for Young Carers/Parent Carers).
Contribute to Compass House’s achievement of GP Carers Quality Markers and compliance with national and local expectations, including the NHS Long-Term Plan.
Support initiatives such as carers’ contingency planning, coding, identification of young carers, and Carers’ Passport schemes.
Undertake project work as required.
Maintain processes for the regular reporting of statistics (e.g., Health and Wellbeing Checks, coded carers, carers’ flu checks) with due regard to confidentiality and consent.
Work closely with reception staff, social prescribers, and other practice staff to identify carers and ensure appropriate coding and contingency plans.
Promote carers’ support across the practice, including websites, newsletters, notice boards, and information racks.
Liaise with statutory and voluntary agencies, carers networks, and advocate on behalf of individual carers.
Refer carers to specialist information providers (e.g., Signposts for Carers, Bay Benefits, CAB, Age Concern Torbay).
Contribute to evaluating the Carer Support Worker role and associated services to inform future commissioning.
Deliver Carers Awareness Training to practice staff

Requirements:

Experience working with carers, vulnerable adults, or in health/social care settings.
Understanding of the Care Act, carer assessments, and local support services.
Strong communication and interpersonal skills.
Ability to work independently and as part of a multidisciplinary team.
Empathy, patience, and a commitment to improving carers’ wellbeing.
Experience in project work and data reporting is desirable.
