Full job description

To provide and manage a Consultant supervised cardio pulmonary exercise testing (CPET) facility in the preadmission assessment clinic to risk stratify and optimise patient fitness before surgery

The post holder will be responsible for implementing, co-ordinating and delivering effective, high quality care for the pre-operative patient, following a client-centred model of practice and ensuring the area of work runs smoothly and efficiently

To make appropriate verbal and written assessment to gain information relating to a patients surgery

To act on such assessments in preparing and supporting the patient’s pathway, referring to outside specialist help if required

Can show interest/ experience in working as part of an MDT in research as an associate, PI or CI.

Provide high standards of patient care for those patients and their relatives attending the pre assessment department

Use of audit and data collection to demonstrate results of service and ensure these are shared with wider team

To lead by example demonstrating effective role modelling to all members of the team and demonstrate clinical expertise

***Preference will be given to internal Trust staff, as well as ‘Priority’ and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.***

Suitable post for an experienced registered practitioner who wishes to contribute to the work of the Pre-Admission Assessment Clinic through the provision of high-quality exercise testing and pre-assessment services

To contribute to the overall efficiency and development of the pre assessment service.

To demonstrate clinical expertise in the pre-assessment unit

Ensure adherence to the pre-operative assessment policy.

Working with the leads of the pre-assessment service to identify and develop clinical changes to working practice and working in different ways to enhance the service provision and patient experience

To provide daily clinical leadership and management and support for the service.

CLINICAL PRACTICE

Ability to run and co-ordinate the CPET service on a daily basis and acting on such assessments in preparing and supporting the patient’s pathway, referring to outside specialist help if required and escalating concerns as necessary.

Actively monitor and make preliminary assessment of exercise testing.

Applies clinical judgement to identify appropriate clinical and physical interventions which may include a range of options.

Able to translate theory into the practical delivery of care for patients within the pre-assessment service

Recognise and act upon appropriately situations that may be detrimental to the health and wellbeing of the individual

Will be expected to take and interpret clinical information and apply this to ensure individual patient needs are met.

Ability to complete an accurate and comprehensive pre-assessment of patients requiring surgery by undertaking a targeted history ensuring they are fully prepared physically, psychologically, emotionally and socially for their procedure

Advise patients on the promotion of health and prevention of illness

Collates and analyses information on patient treatment plans/timescales, identifying bottlenecks and developing action plans to be implemented with the support of the multi-disciplinary team

Works with the multi-disciplinary team to support the organisation and monitoring of the patient journey in order to provide continuity of patient care.
