The Milton Keynes Adult Community Speech and Language Therapy Service is a friendly, innovative and highly supportive team of therapists and support staff who are passionate about delivering high-quality Speech and Language Therapy at the point of need.
An exciting opportunity has arisen for a motivated and enthusiastic Band 6 Community Speech and Language Therapist to join the team, working within the Home1st Planned Care and Domiciliary Speech and Language Therapy pathways.
The successful candidate will work with adults requiring Speech and Language Therapy input in their usual place of residence, including their own homes and residential or care settings. The role will include working with patients who may require more urgent intervention through the Home1st pathway, as well as those requiring SLT-specific input through the Domiciliary pathway.
This post would suit a Band 6 Speech and Language Therapist who has experience in independently assessing and managing adults with dysphagia and communication difficulties.

The post holder will have experience in providing specialist assessment, treatment and management for adults with acquired speech, language and communication needs, as well as eating and drinking difficulties, that maybe due to neurological and progressive conditions, age related or due to other health conditions in a range of community settings.
The role requires excellent clinical reasoning, communication, organisational and caseload management skills, alongside the ability to work independently, lone work safely, contribute to multidisciplinary working, and deliver high-quality, person-centred care.
The post holder will also contribute to service development, clinical training, supervision of less experienced staff, assistants and students, and the development of care pathways and service information for patients, carers and professionals.
The role will require the ability to work independently, self-motivate, show initiative, manage competing demands and work safely within a pressured community environment.
Due to the community-based nature of the role, applicants must hold a full UK driving licence and have access to a vehicle for work purposes.

The post holder will manage referrals into the Home1st and Domiciliary SLT pathways, including triage, prioritisation and waiting list management.
The post holder will assess, develop, implement and evaluate specialist Speech and Language Therapy interventions for adults with acquired communication and swallowing disorders.
The post holder will provide specialist advice to patients, relatives, carers, non-specialist staff and other professionals regarding the management and care of adults with communication and swallowing difficulties.
The post holder will involve clients, relatives and carers in planning and prioritising care plans and will contribute to multidisciplinary planning to support identified goals and outcomes.
