This post will form part of a developing Welsh Intensive, Intervention and Risk Management Service, (IIRMS). In the course of their duties will be responsible for;
- Providing positive intervention during transitional periods that can enhance the success of offender rehabilitation, leading to reduced rates of reoffending and sustained periods of stability in the community.
- Improving access and progression through services by supporting transitions between clinical services in secure OPD sites in England and clinical services in Wales, also to improve maintenance of contact between community services and offenders when recalled to custody.
- Coordinating with key agencies (statutory and Voluntary) within the prison community and the wider geographical community to meet the complex needs of the identified client group.
- Fostering and developing working and partnership arrangements at relevant groups, meetings, forums providing specialist advice, knowledge on the Transition Offenders.

Communication
- Provide support to improve service users’ access and progression through services by supporting transitions between prison, Approved Premisses and 'home' community locality including practical support to individuals to access services, attend appointments, complete relevant forms etc. Ensuring that all sensitive confidential information is communicated appropriately with parties upon the transition of any service user to existing community services, especially on matters relating to previous offending/dangerous behaviour and their risk management.
- Ensure effective sharing of information about risk and risk management of service user/s throughout the transition to ensure a successful and safe transition which will also include engaging with local services to develop individual engagement/ transitional plans. That support the facilitation of local/home service providers' understanding of the needs/presentations of the service user/s to enhance positive outcomes in relation to GP registration, permanent housing, sustainable employment opportunities etc. providing emotional, relational, and practical support for challenges which will present themselves during this transitional period.
- Work closely with an array of services including HMPPS, other statutory and third sector / independent services assist secondary mental health services (where applicable) with specialist and at time complex advice and support regarding the management of service users on their caseloads who are eligible for the OPDP.
- Contribute to the development of more effective pathways through attendance at CTP, sentence planning, MAPPA, and MARAC meetings; bringing professionals together; service mapping; awareness raising; facilitating communication and referrals between services; and assisting with 'unpicking' of blockages. Increasing quality of clinical services by supporting the maintenance, generalisation to the community, and the continued development of therapeutic gains; facilitating the communication of information about risk and risk management by enhancing the relational and practical support during the difficult transition from approved premises to the community.
- Regularly prepare for, attend and participate in multi-professional meetings and other meetings where the service user's progress is discussed as well as collaborative goal setting with service users, other professionals, family members and other support sources whenever possible.
Planning and Design
- Co-ordinate annual leave with colleagues to ensure the continued best functioning of the department.
- Coordinate and support connections between existing statutory and third sector service providers — substance misuse, support networks, service user groups, Citizens Advice Bureau etc. in collaboration with OPDP colleagues of service users who are in custody awaiting transfer to Approved Premises that meet the criteria for the OPDP pathway.
- Schedule and prioritise sessions working from various Probation Offices within the South Wales area, liaising with Probation Practitioners on a face-to-face basis regarding complex cases.
Participate in the development and implementation of robust contingency planning, particularly in relation to crisis intervention.
- Autonomously manage own caseload ensuring that travel between treatment sites is most effective.
Improvement, Monitoring, Policy/Service Development
- Adhering to professional Code of Professional Conduct ensure that a high standard of provision and professionalism is maintained during procedures and interventions implemented are compliant with legal implications of working with service users in the community.
- Contribute to the implementation of NICE Guidelines, National Service Frameworks and the professional leadership agenda and adhere to Professional and organisational guidelines governance and record keeping e.g. GDPR.
- Comply with all SBU Health Board policies and procedures Legislation and Strategy requirements e.g. Health and Safety.
- Assist and participate in planning the future developments of the service. Incorporating service user's perspectives within service development wherever possible.
Clinical
- Assist the identification of high risk OPDP eligible service users who present with particularly complex needs with their access to appropriate services, resources, treatment and/or support networks within their identified home area.
- Provide referrals for support in key areas such as housing, substance misuse interventions, educational opportunities, participation in voluntary and/or permanent work by means of outreaching from the approved premises, and where appropriate access to Mental Health Services where difficulties may be encountered as part of the CTP framework. To identify the exact nature of the needs as identified by service users and staff using the Goals and Plans framework alongside information from probation / psychology reports.
- Identify/facilitate short-term evidence-based interventions working in a trauma informed, recovery focused manner that can be implemented during transition, such as anxiety management, emotional regulation, etc.
- Provide strategic and practical support to service users by way of creating a service map and resource / contacts guide as a means to establish links to local services including GPs; housing providers; employment services; primary care; substance misuse services; local support groups etc. Participate in the service's response for consultation, formulation and assessment of clients with complex and high-risk behaviours, who are managed by other agencies within the criminal justice system.
- Ensure that the physical health needs of specific client group are adequately achieved through collaborative management with Primary Care providers, particularly for those individuals who would normally neglect this aspect of their health needs.
Management, Training & Leadership
- Participate in the recruitment process, interviewing of staff, PADR and regular supervision/appraisal both as a supervisee and supervisor of practitioners within the Transitional Team and delegate duties to Practitioners within the Transitional Team.
- Take an active role in the ongoing education and training of students, nursing and other professional staff within the service inaccordance with the declared aims and objectives of the placement assessing competencies required by their placement outcomes.
- Participate in induction training for all new professionals within the OPDP team. Offer hub and spoke placements, induction placements for professionals within the criminal justice team.
- Actively seek opportunities to continually develop oneself, consistent with the principles of lifelong learning and in line with your professional bodies'.
- Complete all statutory & mandatory training within required time frames before requesting any additional training in respect of continued professional development.
Finance and Budget
- Ensure that department furniture and equipment is maintained in good order and arrange for repair and replacement when necessary.
Digital and Information
- Develop and provide statistical data/reports related to own practice.
- Maximise the use of information technology for the storage of data and benefit of department and service development.
Research Development. Evaluation & Audit
- Participate and implement a system for monitoring and auditing practice.
Independence Initiative
- Make referrals to the statutory services, voluntary social services, and other professional bodies.
- Apply departmental code of safety to your own practice, incorporating lone working policy and arrangements for the recording of untoward incidents as appropriate.
- Act as a supervisee, accessing managerial, clinical and peer supervision, when required, act as a supervisor.
Security
- Take all possible precautions to safeguard the welfare and safety of staff, patients, visitors and the public, by implementing all policies and procedures relating to security.
- Ensure that any intervention is implemented in accordance with service users leave and legal status and take appropriate measures to ensure personal safety during interventions.
