Overview
We are seeking a dedicated Care Navigator / Administrator to join our healthcare team. This vital role involves coordinating patient care, managing administrative tasks, and ensuring smooth communication between patients and healthcare providers. The ideal candidate will possess strong organisational skills, excellent communication abilities, and previous office experience. This position offers an opportunity to contribute meaningfully to patient well-being within a supportive environment.

Responsibilities

Act as the primary point of contact for patients, providing guidance and support throughout their care journey
Organise appointments, follow-ups, and referrals in accordance with healthcare protocols
Maintain accurate and up-to-date patient records using electronic health record (EHR) systems
Coordinate with multidisciplinary teams to facilitate seamless patient care delivery
Manage administrative duties such as data entry, filing, and correspondence
Assist in preparing reports and documentation for internal and external stakeholders
Ensure compliance with data protection regulations and confidentiality standards
Support the continuous improvement of administrative processes to enhance efficiency

Experience

Prior office or administrative experience is essential, preferably within a healthcare or social care setting
Familiarity with healthcare systems, patient management software, or EHR platforms is advantageous
Excellent organisational skills with the ability to prioritise tasks effectively
Strong verbal and written communication skills in British English
Ability to work independently as well as part of a team in a fast-paced environment
Compassionate approach with a focus on delivering exceptional patient service
