Full job description

We are seeking a highly skilled and motivated Advanced Nurse Practitioner (ANP) to join our established, patient‑centred general practice team. The post‑holder will deliver advanced autonomous clinical care, supporting patients with complex needs, frailty, acute presentations, and long‑term conditions. You will work as part of a multidisciplinary team, contributing to safe, effective and high‑quality care across the practice and Primary Care Network.

The successful candidate will undertake comprehensive clinical assessments, including history taking, examination, diagnosis and management, and will act as an independent prescriber. You will lead and support same‑day/acute triage, manage urgent presentations, recognise deterioration, and ensure timely escalation and referral. The role includes delivering chronic disease clinics, personalised care planning, medication reviews, and preventative health interventions.

You will contribute to clinical governance, quality improvement, safeguarding, and achievement of QOF and enhanced service targets. As a senior clinician, you will provide leadership, mentorship, and support workforce development within the team.

Applicants must hold NMC registration, evidence of advanced‑level practice, an MSc, and the V300 prescribing qualification. Experience in general practice, chronic disease management, autonomous decision‑making, and clinical governance is essential.

Summary of Key Duties

Advanced clinical assessment – autonomous history taking, examination, diagnosis and management
Acute & same‑day triage – telephone, online and urgent clinic triage
Independent prescribing – safe prescribing, medication reviews and optimisation
Frailty & complex care – lead on frailty, acute care and end‑of‑life support
Chronic disease management – diabetes, asthma, COPD, hypertension
Clinical leadership – mentoring, supervision, service development
Governance & QI – audits, SEAs, safeguarding, CQC compliance
MDT collaboration – PCN working, integrated care pathways

Pay: £49,387.00-£56,515.00 per year
