We are looking for a compassionate and motivated Senior Perinatal Community Practitioner to join our Specialist Perinatal Mental Health Team. Based primarily within the York and Selby locality, you will provide specialist assessment, intervention, and support for women and birthing people experiencing moderate to severe mental health difficulties during pregnancy and up to the baby's first year.
As a senior team member, you will work closely with families, maternity services, primary care, and partner agencies to deliver person-centred care. You will also contribute to service development, promote best practice, and support colleagues in providing excellent care.
Although mainly based in York and Selby, the role requires travel across the Trust footprint to support service delivery, attend meetings, training, and development opportunities.

As a Senior Perinatal Community Practitioner, you will play a key role in delivering high-quality specialist mental health care to women and birthing people experiencing moderate to severe mental health difficulties during the perinatal period.
Key responsibilities include:
- Undertaking specialist assessments
- Developing, implementing, and reviewing evidence-based care plans
- Delivering specialist therapeutic interventions and coordinating care within a community setting.
- Providing consultation, advice, and support to professionals across maternity services, primary care, health visiting, social care, and voluntary sector organisations.
- Working collaboratively with women, birthing people, families, carers, and partner agencies to promote positive outcomes for both parent and infant.
- Contributing to safeguarding processes and ensuring the safety and wellbeing of service users, infants, and families.
- Acting as a lead professional where appropriate and maintaining responsibility for a defined caseload of service users with complex needs.
- Ensuring care is delivered in a trauma-informed, culturally sensitive, person-centred, and inclusive manner that values diversity and promotes equality.
- Contributing to service development, quality improvement projects, audit activity, and the implementation of evidence-based practice
