Overview

Join our compassionate team as a Care Manager, where you will play a vital role in enhancing the quality of life for our residents and clients. In this dynamic position, you will coordinate personalized care plans, oversee daily care activities, and ensure that each individual receives respectful, attentive, and professional support. Your expertise will help foster a safe, engaging, and nurturing environment for seniors and individuals with complex health needs. This role offers an exciting opportunity to make a meaningful difference while working in a supportive and energetic setting.

Responsibilities

Develop, implement, and regularly update comprehensive care plans tailored to each resident’s unique needs, including medical, emotional, and social aspects
Coordinate with healthcare professionals to ensure proper medication administration and adherence to treatment protocols
Monitor residents’ behavior and condition, employing effective behavior management techniques to promote well-being
Provide direct patient care such as assistance with activities of daily living, mobility support, meal preparation, and light housekeeping tasks
Conduct regular assessments to evaluate progress and adjust care strategies accordingly
Maintain accurate documentation of care activities, medication records, and incident reports in compliance with regulatory standards
Support the management team as part of the on-call rota

Qualifications

Proven experience in nursing home settings or senior care environments with a strong understanding of dementia and Alzheimer’s care
Certification or training in first aid is highly desirable
Knowledge of medical terminology and case management principles to coordinate effective care delivery
Experience home care services is essential.
Ability to perform heavy lifting safely when assisting residents with mobility or transfers
Valid driver’s license with reliable transportation for community visits or appointments
Excellent communication skills to collaborate effectively with residents, families, and healthcare teams
