This is a varied office and community-based operational role and is ideal for someone who enjoys working in a fast-paced environment where every day is different.
You'll be responsible for coordinating care services, supporting Care Workers, completing client assessments, maintaining compliance and ensuring our clients receive outstanding care.
This is not a traditional office administration role. Whilst there is office-based work, you will regularly work within the community carrying out assessments, quality assurance visits and supporting Care Workers.
As a domiciliary care provider operating between 7:00am and 10:00pm, seven days a week, flexibility is essential. Although your standard shift is 9:00am–5:00pm, additional hours will occasionally be required to support the service during emergencies, staff shortages or periods of high demand.
Every effort will always be made to source cover for care visits. However, where cover cannot be found, Care Coordinators are expected to deliver care themselves to ensure our clients continue to receive safe, high-quality care.

Main Responsibilities
The Care Coordinator is responsible for supporting the effective day-to-day operation of the domiciliary care service.
Responsibilities include, but are not limited to:
Care Coordination
- Managing a live rota and responding quickly to changes throughout the day.
- Scheduling Care Workers to ensure all visits are safely covered.
- Managing emergency changes due to sickness, annual leave and unforeseen circumstances.
- Monitoring staff availability and service delivery.
- Ensuring continuity of care wherever possible.
Client Management
- Completing initial care assessments.
- Producing and reviewing care plans.
- Completing and reviewing risk assessments.
- Carrying out regular client reviews.
- Responding professionally to client queries, concerns and complaints.
- Liaising with families and healthcare professionals.
Staff Support
- Supporting Care Workers with day-to-day operational issues.
- Conducting supervisions.
- Carrying out competency observations and spot checks.
- Recruiting, interviewing and onboarding new Care Workers.
- Supporting staff development and performance.
Quality & Compliance
- Maintaining accurate electronic records.
- Ensuring compliance with CQC Fundamental Standards.
- Monitoring quality across the service.
- Supporting audits and quality assurance processes.
- Promoting best practice across the organisation.
Operational Responsibilities
- Participating in the company on-call rota (currently one week in every two).
- Responding to emergencies outside normal working hours whilst on-call.
- Providing direct care where emergency cover cannot be sourced.
- Supporting the management team with the day-to-day running of the service.
- Undertaking any other duties appropriate to the role.
Working Pattern
This is a full-time position working across a 7-day rota.
- Standard working hours are 9:00am–5:00pm.
- You will work 5 days out of 7, with 2 days off each week according to the rota.
- Participation in the on-call rota is required (currently one week in every two).
- Our service operates between 7:00am and 10:00pm, 365 days a year.
- Flexibility is essential.
- Additional hours may be required during busy periods, emergencies or staff shortages.
- When operationally required, Care Coordinators are expected to provide direct care if alternative cover cannot be arranged.
Salary & Benefits
- £13.33 per hour, plus statutory holiday pay (equivalent to £14.94 per hour including holiday pay).
- £42.86 on-call payment for each day you are scheduled to be on-call.
- Any care visits completed outside of your normal working hours (9:00am–5:00pm) are paid at your hourly rate, in addition to your daily on-call payment.

Essential Requirements
- Full UK Driving Licence.
- Access to your own vehicle.
- Willingness to obtain business insurance.
- Minimum two years' experience working within Health & Social Care.
- NVQ/QCF Level 3 in Health & Social Care (or equivalent), or NVQ/QCF Level 2 with a commitment to complete a fully funded Level 3 qualification following successful completion of your probationary period.
- Willingness to travel throughout the local community.
Knowledge, Skills & Experience
The successful candidate will:
- Have previous experience within Health & Social Care.
- Have previous experience as a Care Coordinator, Senior Carer, Team Leader or similar role, or be ready to progress into care coordination.
- Be highly organised and able to prioritise multiple tasks.
- Remain calm under pressure.
- Be confident making decisions.
- Have excellent communication and customer service skills.
- Enjoy problem-solving.
- Work well independently and as part of a team.
- Have experience using electronic rota planning and care management systems.
- Have a good understanding of CQC regulations and compliance.
- Demonstrate professionalism, integrity and accountability at all times.
