Job Overview

We are seeking a compassionate and proactive Care Navigator to join our dedicated healthcare team. In this vital role, you will serve as a guiding resource for patients, helping them navigate complex healthcare systems, access necessary services, and understand their treatment options. Responsibilities

Act as the primary point of contact for patients seeking assistance, providing clear guidance and support their healthcare experience
Coordinate appointments and referrals to community resources
Maintain accurate and detailed documentation of patient interactions
Collaborate with healthcare providers and support staff to ensure seamless communication and continuity of care
Educate patients about available services, eligibility criteria, and how to access various health programs

Qualifications

Prior experience working in an office environment or administrative role within a healthcare setting is highly desirable
Strong organizational skills with the ability to manage multiple tasks efficiently
Excellent communication skills—empathetic listening and clear verbal/written expression are essential
Ability to navigate healthcare systems and understand basic medical terminology
Demonstrated ability to work independently while collaborating effectively with a multidisciplinary team
Proficiency in office software applications such as Microsoft Office Suite, SystmOne and other similar tools
