Full job description

Care Coordinator

At Quality Care (SC), being a Care Coordinator is about much more than managing rotas. You'll play a vital role in delivering high-quality, person-centred domiciliary care by coordinating care services, supporting our care team, and ensuring our clients receive safe, responsive, and compassionate care.

This is a varied and rewarding role, combining office-based coordination with community-based responsibilities. In addition to coordinating care, you will undertake client assessments, care plan reviews, quality assurance visits, spot checks, and provide practical support to our care staff when required.

As this role involves regular travel within the community, you must hold a full UK driving licence and have access to your own vehicle.

Key Responsibilities

Plan, organise, and manage weekly care rotas to ensure efficient and effective service delivery.
Schedule care visits while considering continuity of care, travel times, and carers' availability.
Ensure adequate staffing levels and arrange cover for sickness, annual leave, and emergency absences.
Conduct new client assessments, care plan reviews, and risk assessments.
Support the Registered Manager with the implementation of new care packages and the allocation of suitable care staff.
Build positive working relationships with clients, families, social workers, GPs, district nurses, and other healthcare professionals.
Respond professionally and compassionately to enquiries from clients, families, and care staff.
Carry out spot checks, service user visits, telephone monitoring, and other quality assurance activities.
Participate in the additionally paid on-call coordinator rota, on an occasional basis, to cover annual leave, sickness, or unexpected emergency situations for on call. This is not a regular responsibility but may be required from time to time to help ensure continuity of care and maintain high-quality service delivery.
Maintain accurate electronic and paper-based records in accordance with company policies and Care Inspectorate requirements.
Contribute to continuous service improvement and support the ongoing growth of the organisation.

Essential Requirements

At least 1 year's experience as a Care Coordinator within a domiciliary or home care setting.
Full UK driving licence and access to your own vehicle.
Excellent organisational, communication, and time-management skills.
Strong IT skills and confidence using electronic care management systems.
Ability to prioritise workload, solve problems, and work effectively under pressure.
A professional, compassionate, and person-centred approach.
Reliable, trustworthy, and able to work independently using your own initiative.

Desirable

SVQ Level 3 in Health & Social Care (or equivalent).
Previous experience of quality assurance, assessments, and care reviews.
Knowledge of Care Inspectorate standards and regulatory requirements.
