Are you passionate about helping people live independently and achieve the outcomes that matter most to them? We are looking for two experienced and motivated professionals to join our Localities Team, supporting adults with a wide range of complex needs across the Isle of Wight. This is a rewarding opportunity to make a real difference by working alongside individuals, families, carers and partner organisations to promote independence, wellbeing and community inclusion.

In this varied role, you will manage a complex caseload and undertake comprehensive assessments, care planning and reviews, ensuring your practice is person-centred, strengths-based and outcome focused. You will work within key legislative frameworks including the Care Act, Mental Capacity Act and Human Rights Act, helping people access the right support at the right time while safeguarding those most at risk. Your work will involve responding to urgent situations, undertaking crisis intervention, supporting safeguarding enquiries, and contributing to decisions that ensure resources are used effectively and appropriately.

Alongside direct casework, you will play an important role in supporting colleagues across the service. This includes mentoring newly qualified or junior staff, sharing professional expertise, providing guidance on complex cases and contributing to a culture of reflective practice and continuous improvement. You will also participate in an out-of-hours emergency support rota, providing crisis assistance to Island residents when needed.
