JOB DESCRIPTION

Post: Wellbeing Support Worker

Employment Terms: One year fixed contract

Reporting to: Line Manager

Line Management Responsibility: Volunteers

Hours per week: 37.5 hours

Salary Scale: £31,713.50

Overall purpose of the job

To deliver varied wellbeing programmes designed to promote motivation and the ability to live a full, healthy lifestyle and engage in meaningful activities. This will involve partnership working with agencies, linking residents to services for specific support. The role will involve building trust and assessing residents needs to ascertain appropriate support.

You’ll work as part of a holistic team to research, plan, organise and facilitate a holistic wellbeing programme to be delivered to enable rough sleepers to improve their mental health, physical and emotional wellbeing.

Collaborating with local agencies will be a vital component of the role to deliver maximum impact in helping residents sustain their accommodation and achieve a better quality of life.

To begin to build a therapeutic environment centred around the resident’s input with a focus on community.

Working hours: 37.5 hours weekly Monday to Sunday between the hours of 8am and 9pm including weekends, and bank holiday working. Actual days, start/finish times maybe variable and in accordance with the service rota.

In collaboration with the senior management, the post holder will take overall responsibility for:

· Link residents into the community using a holistic approach engaging them to improve their health and wellbeing. Striving to make a positive impact on people’s lives and tailor support to enable people to fulfil their best potential.

· Build and maintain professional relationships with external partners and stakeholders.

· Identify areas of support within the community and mental health services that supports the residents’ goals.

· Deliver and record outcomes that the funding stipulates.

Main duties & responsibilities:

· Create the planning and delivery of a variety of social, leisure and recreational activities and opportunities based on identified need. Developing joint activities and opportunities where possible.

· Support residents to access a wide range of activities that increase self-confidence, improve health & wellbeing and develops communication and social skills.

· Develop positive and trusting relationships with residents to support them to fully engage in activities and to remain as independent as possible.

· You will help provide outcome focused, person centred support, empowering people to become active members of their community.

· Support individuals to increase their life skills involving a variety of housing support needs.
Promoting communication is essential, so individuals can meaningfully participate in decisions and choices that matter to them, whilst expressing their own personality and talents.

· To support our residents who may feel socially excluded in society to gain confidence and self-esteem to feel included in their community.

· To promote positive mental and physical wellbeing through preventative and early intervention approaches. With the aim of equipping people with the skills needed to live independently and meaningfully within the local community, a strengths-based coaching approach to recovery is used to build confidence, maintain motivation and promote resilience.

· Provide practical support with food preparation, budgeting and other daily living skills. Promoting participation and inclusion is a core value of the service, through working alongside people to deliver innovative, creative and personalised interventions and opportunities.

· To actively engage with residents in order to deliver a person-centred service that is meaningful and valued.

· Motivating clients to engage with and take ownership of their health and wellbeing.

· Line management of volunteers.

· Attend to all administrative duties related to the post.

· Align with CHESS Core Values – Respect, honesty, team-player, Solution-focused, empathy and enthusiasm.

· Take responsibility for personal training, development and attend appropriate supervision & training sessions.

Person Specification

Essential

· Experience of supporting adults using a person-centred approach

· Experience of facilitating and delivering a range of activities

· Identifying individualised needs and outcome planning.

· Knowledge of the social care sector.

· Knowledge of the Voluntary Sector

· Interpersonal skills and the ability to build and develop relationships.

· A collaborative approach to work

· Honest, respectful, reliable, non-judgmental.

· Awareness of professional boundaries

· Flexibility with working hours

· Good working knowledge of Microsoft Office & CRM system

· Ability to be creative when working with residents.

You will be required to have a full UK driving licence for this role

Desirable

· Ability to deal with challenging behaviour with a non-confrontational approach.

· Awareness of the need for outcomes
