Job Overview

We are seeking a dedicated and proactive Care Co-ordinator to join our dynamic healthcare team. In this vital role, you will oversee the coordination of care services for clients in various settings, ensuring personalized, safe, and effective care plans are implemented. Your leadership will help foster a compassionate environment where clients feel supported and valued. The ideal candidate will possess strong communication skills, supervisory experience, and a passion for senior and care home environments. Responsibilities

Develop, review, and update comprehensive care plans tailored to each client’s individual needs and preferences
Coordinate with care teams, families, healthcare professionals, and service providers to ensure seamless delivery of care services
Supervise and support care staff, providing guidance on best practices and ensuring adherence to care standards
Monitor client progress regularly, adjusting care plans as necessary to optimize well-being and safety
Oversee transportation logistics, including driving clients to appointments or activities when required
Maintain accurate documentation of all care activities, incidents, and client interactions using IT systems
Lead on safeguarding procedures, ensuring all staff follow protocols to protect vulnerable clients

Skills

Proven experience supervising or managing care teams within a senior or care home setting
Strong leadership qualities with the ability to motivate and guide staff effectively
Excellent communication skills for liaising with clients, families, healthcare professionals, and team members
Competence in developing and managing detailed care plans that meet regulatory standards
Valid driving license with the ability to drive clients safely when needed
Proficiency in using IT systems for documentation, scheduling, and reporting purposes
Knowledge of safeguarding policies and procedures relevant to vulnerable adults
