Social Care Facilitator – 3 Month Contract

Location: Walsall

Rate: £22.91 per hour (Umbrella)

Contract: 3 Month Contract

Hours: Full Time

Working Pattern: Hybrid – Minimum 2 Days Office Based

Enhanced DBS Required

Full UK Driving Licence Required

Summary

We are seeking a dedicated Social Care Facilitator to join our dynamic Adult Social Care team for a 3-month contract. This role is pivotal in supporting vulnerable adults and their carers by assessing needs, aiding in care planning, and promoting independence. As part of a multi-disciplinary team, you will ensure that individuals can access the necessary services and community resources to achieve positive outcomes. This position is ideal for someone with a background in Adult Social Care, Care Management, Support Planning, or Community-Based Services.

Responsibilities

Support the assessment of vulnerable adults and their carers.
Contribute to support plans that promote independence, choice, and wellbeing.
Assist service users in accessing community resources and support services.
Undertake allocated care management duties.
Work in a person-centred way with service users, families, and carers.
Support individuals to maintain choice and control while considering mental capacity.
Collaborate with social workers, healthcare professionals, and partner agencies.
Identify the need for assistive equipment and arrange provision where appropriate.
Refer individuals for specialist assessments where complex equipment or adaptations are required.
Maintain accurate records and update case management systems.
Participate in team meetings, training, and professional development activities.

Requirements

Requirements:

Experience working within Adult Social Care, Care Management, Support Planning, or a similar setting.
Experience supporting vulnerable adults and carers.
Knowledge of person-centred practices and promoting independence.
Experience working within multi-disciplinary teams.
Strong communication and interpersonal skills.
Ability to manage a caseload and prioritise workloads effectively.
Good ICT and record-keeping skills.
Full UK Driving Licence.
Enhanced DBS.

Desirable

Local Authority Adult Social Care experience.
Knowledge of assistive equipment and adaptations processes.
Understanding of mental capacity and safeguarding principles.
Experience working with community and voluntary support services.
