Clinical Pharmacist

One Ashford Hospital | FlexiBank - Hours as and when required.

We are seeking a dedicated Clinical Pharmacist to join our Pharmacy team on a FlexiBank arrangement at One Ashford Hospital, part of Phoenix Hospital Group. This role offers a unique opportunity to provide pharmaceutical care and support to patients, ensuring the safe and effective use of medications. As a Clinical Pharmacist, you will play a vital role in enhancing patient outcomes and collaborating with healthcare professionals in a dynamic medical environment.

Responsibilities:

Conduct comprehensive medication reviews and assessments for patients.
Provide expert advice on medication management to healthcare professionals.
Monitor patient responses to medications and adjust treatment plans as necessary.
Educate patients on the safe and effective use of their medications.
Collaborate with multidisciplinary teams to optimise patient care.
Maintain accurate and up-to-date patient medication records.
Participate in clinical audits and quality improvement initiatives.

Qualifications:

Bachelor's degree in Pharmacy or equivalent.
Registered with the General Pharmaceutical Council (GPhC).
Minimum of 1 year of experience in a clinical pharmacy setting.
Strong knowledge of pharmacology and medication management.
Excellent communication and interpersonal skills.
Ability to work effectively in a team and independently.
Proficient in using pharmacy management systems and electronic health records.
