Westbourne Medical Centre is a thriving and forward-thinking GP practice located in the heart of Westbourne, Bournemouth (BH4 9HJ), serving a growing patient population. At Westbourne Medical Centre, we believe that digital technology has the power to make healthcare more accessible, personalised, and efficient for everyone. We are looking for a Digital Care Coordinator to help patients confidently engage with digital healthcare services and support them in making the most of the tools available to manage their health and wellbeing.

This is an exciting opportunity for someone who is passionate about healthcare, technology, and improving patient engagement. The successful candidate will promote the use of digital health solutions, including NHS apps and remote monitoring tools, empowering patients to take a more active role in managing their physical and mental wellbeing both at home and on the move. By supporting patients to develop their digital confidence, you will play a key role in helping them access the right care, at the right time, through the right channels.

This is a full-time position of 37.5 hours per week, worked Monday to Friday. We offer flexibility in working patterns, with hours scheduled between 8:00am and 6:30pm to meet both service and individual needs. Final shift pattern will be agreed with the successful candidate to ensure adequate practice cover while supporting a healthy work–life balance.

Main duties of the job

You will be enthusiastic, meticulous, and proactive. Keen to work as part of a team and self-motivated with well-developed communication skills. You will have excellent IT literacy but might not yet have experience of working in health or care settings. Nonetheless, you will be open to learning new skills and ideas, and to developing the care co-ordination role to its maximum potential.

Job responsibilities

Digital Inclusion and Patient Support

Identify patients who may benefit from support to access and utilise digital healthcare services.
Support individuals and groups experiencing digital exclusion to improve their digital skills, confidence, and access to healthcare services.
Provide ongoing support to patients who are already using digital healthcare technologies and services.
Promote the use of NHS digital services, including online consultation platforms, the NHS App, and other approved digital healthcare tools.
Work collaboratively with partner organisations, voluntary sector services, and community groups to promote digital inclusion and improve access to healthcare services.
Act as an ambassador for digital healthcare, engaging with patients, carers, and families to encourage the effective use of digital tools and services.
Complete administrative tasks and data entry to facilitate patient access to online healthcare services.

Digital Projects and Service Improvement

Support the PCN Management Team with projects relating to digital transformation, digital tools, health inequalities, and health literacy.
Contribute to the development and delivery of ongoing and new programme initiatives across the PCN.
Work collaboratively with colleagues to support digital engagement activities, service development initiatives, and improvements to patient access and experience
Support the collection, analysis, and reporting of data to inform service development, population health management, and patient engagement initiatives.

Data Management and Clinical Pathway Support

Undertake data entry and administrative tasks to support PCN programmes, audits, and reporting requirements.
Support cardiovascular disease prevention and management programmes, including blood pressure monitoring, reporting requirements, and patient engagement activities.
Assist with patient recall systems for long-term condition management, helping to ensure patients receive appropriate monitoring and follow-up care.
Support the identification and engagement of patients eligible for targeted health initiatives and preventative healthcare programmes.

Patient Engagement and Communication

Manage and develop the online Patient Participation Group (PPG), encouraging patient participation, gathering feedback, and promoting engagement with primary care services.
Monitor online activity, create patient engagement content and survey, and respond to comments and messages in a timely and professional manner.
Support initiatives that strengthen patient engagement and improve communication between patients and primary care services.

Person Specification

Personal Qualities & Attributes

Confident and comfortable with the use of digital healthcare technology and local data and intelligence systems

Ability to organise, plan and prioritise on own initiative, including when under pressure.

Ability to work as part of a team.

Ability to support people in a way that inspires trust and confidence, motivating others to reach their potential.

Ability to communicate effectively, both verbally and in writing, with people, their families, carers, partner agencies and stakeholders.

Ability to maintain effective working relationships and to promote collaborative practice with colleagues.

Able to collaborate effectively with their peer group, to share best practice and offer support and shared learning.

Experience

Experience in use of databases

Experience of using word, excel and PowerPoint including ability to use word processing skills, emails and the internet to create simple plans and reports.

Experience of working with or in general practice (Desirable)

Knowledge of clinical system “SystmOne” (Desirable)
