About Rhythmic Care

Rhythmic Care is a trusted care provider committed to delivering high-quality, person-centred support to individuals within their own homes (supported living environments). We are currently seeking experienced, compassionate, reliable, and dedicated Support Workers to join our growing team in the Royal Borough of Kensington and Chelsea (SW10).

This is an excellent opportunity for individuals who are passionate about supporting people with learning disabilities and/or Autism and/or complex needs, helping them to live their best life, as independently as possible, safely, with dignity and to achieve their goals.

About the Role

As a Support Worker, you will play a vital role in supporting young people/adults with learning disabilities/Autism/complex needs and housing-related care needs. You will use a Strengths-Based, person-centred approach to encourage independence, promote choice and inclusion, supporting health and wellbeing and provide practical and emotional support tailored to each individual's needs.

Every day is different, and your work will have a direct positive impact on the lives of the people you support.

Key Responsibilities

Your duties may include:

· Assist individuals with daily living activities according to their support plans.

· Support with personal care where required, including bathing, dressing, grooming, and mobility.

· Encourage and promote independence in all aspects of daily living.

· Support individuals to attend medical appointments.

· Assist with medication prompting and administration (ad required) and recording in accordance with company policies.

· Monitor and report any changes in physical or mental health.

· Providing emotional support and companionship

· Support individuals with meal planning, shopping, and food preparation.

· Assist with household tasks such as cleaning, laundry, and maintaining a safe living environment.

· Help service users manage finances and budgeting where appropriate.

· Preparing meals and supporting with nutrition and hydration

· Encourage participation in social, educational, and recreational activities.

· Support individuals to access community facilities and maintain relationships with family and friends.

· Promote inclusion and equal opportunities.

· Follow safeguarding procedures to protect vulnerable adults from abuse and neglect.

· Maintain accurate records, reports, and documentation.

· Comply with all health and safety regulations, policies, and procedures.

· Maintain confidentiality and professional boundaries at all times.

· Reporting any concerns in line with mandatory training and company policies

· Work collaboratively with colleagues, families, healthcare professionals, and external agencies.

· Participate in team meetings, supervision sessions, and training.

· Contribute to the continuous improvement of the service.

Requirements

We welcome applications from experienced Support Workers (Supported Living) looking to start immediately.

Essential qualities include:

A valued based, caring, compassionate, and patient approach
Good communication and interpersonal skills
Reliability, professionalism, and commitment
Basic literacy, numeracy, and record-keeping skills

· Ability to work independently and as part of a team

· Commitment to promoting independence, choice and control for service users

Willingness to undertake an Enhanced DBS check

· Flexible approach to working hours, including evenings, weekends, and bank holidays.

Desirable

Previous experience working with adults with learning disabilities/Autism/complex needs
Experience within supported living, or housing support services
NVQ/QCF Level 2 or 3 in Health and Social Care (or willingness to work towards one)

Shifts Available

· Work takes place in service users' homes and within the local community.

· May involve lone working and supporting individuals with varying levels of need.

We offer a variety of flexible shift patterns, including:

8-hour, 10-hour, and 12-hour shifts
Day shifts

Weekday and weekend work
Bank holiday shifts
