Full job description

An exciting opportunity has arisen to join a new phase of the Complex Care Programme (CCP) in the South London Partnership- a partnership between Oxleas, South West London and St Georgie’s and South London and the Maudsley NHS Mental Health Trusts. The CCP has a track record of innovation and investing in services to improve the lives of people with complex mental health needs.

SWL ICB are wishing to delegate the placement budget of an additional 300 people with learning disability, autism and older adults to the Programme. This adds to the current portfolio already managing over 1000 mental health placements. Additional commissioning responsibilities will also be delegated.

The post holder will work as part of a new team to

Carry out the placement management function for those in scope, to include screening, assessments, agreeing funding apportionment and review.
Lead on elements of the transformation agenda–co-ordinating functions such as maintenance of Dynamic Support Registers (DSRs), carrying out 8-week commissioner visits, and chairing C(E)TRs (Care, Education and Treatment Reviews).

The successful candidate must have extensive knowledge of and experience of working with people in scope and knowledge of the key legislation, guidance, and policy framework surrounding this service user cohort.

We are seeking individuals who can work independently and work to tight timescales to meet the objectives of the Programme.

As this is a new team, there will be lots of opportunities to shape working practices and service design and think of new ways to collaborate effectively with partners.

The main duties will consist of carrying out placement management functions, involving screening referrals, attending panels, sometimes with Local Authority colleagues. The post holder will be confident in reviewing requests, leading negotiations, as well as working with colleagues to procure new services, including bespoke packages of care.

Secondly, there will opportunities to work more intensively with the people we support and their Network, carrying out 8-week commissioner visits for adults with LD/ASD in inpatient settings, co-ordinating and chairing CETRs.

The postholder will have responsibility for named borough maintenance of the local DSR’s including Special Education Needs (SEN) Teams as appropriate

In their commissioning role, they will review and receive requests for LAEP (Local Area Emergency Protocol), keeping a tracker and chairing meetings as required- playing a role in supporting people to stay out of hospital.

There will also be opportunities to lead on the delivery and implementation of elements of service redesign and transformation programmes. This may involve thinking creatively about developing accommodation and support to bring people closer to home. This will include project planning, implementation, and evaluation where needed.

This post sits in a specialist clinical team of four case managers, all working to a SWL Lead (LD, Autism and Older People), with the support of an administrator. The new team sits within a wider Complex Care Programme, with staff being able to draw on additional expert knowledge in autism, complex emotional needs and complex mental health needs.

A dedicated SLP Commissioning and Contracts Team, and SLP Head of Quality will also support the post holder in their role.

The post holder will be visiting a number of both NHS and private units and may need to visit people out of the local footprint. On occasions they will also need to attend the CCP base in Lewisham, and other sites across South West London and St Georges, and South London and The Maudsley (Croydon).

The post holder is responsible for:
1. Placement Management

With the SWL Lead, establish a referral process and carry out assessments to determine eligibility of access to 100% health only funded placements– eligibility which are consistent with NICE Guidance
To attend funding panels in the local ICB area and act on the delegated nature of the budget.
To review funding requests, agree as necessary and lead the negotiation of funding split allocations with LA and other colleagues
To work with internal partners to procure new services including bespoke packages of care, ensuring best value, increased quality, and price convergence
To manage and provide oversight to placement activity, working with care co-ordinators, key workers etc.
To lead on strategic initiatives relating to Phase 3a placement management
To identify, build and proactively develop effective stakeholder relationships between the SLP trusts’, ICBs’s and LA’s by diplomatically managing the competing priorities that may occur in some environments.

Leading on the transformation agenda for those in scope:

To carry out 8-week commissioner visits for adults with LD/ASD in an inpatient setting,
Co-ordinate, chair and attend (Care (Education) and Treatment Reviews (CETR)
To participate in SWL inpatient case surgery meetings and the CETR oversight panel.
To chair lead the maintenance and oversight of local dynamic support registers (DSR), to ensure effective collaboration between health organisations and local authority partners including Special Education Needs (SEN) Teams as appropriate

To ensure the oversight and reporting of LDA KPIs development and delivery of any required improvement plans.
To review and receive requests for LAEP (Local Area Emergency Protocol), keeping a tracker and chairing meetings as required
To update the Assuring Transformation platform on NHS Digital as required.

To ensure that legislation is adhered to in the care of people with LDA.
To lead on the delivery and implementation of elements of service redesign and transformation programmes for adults with LDA. This will include project planning, implementation, and evaluation where needed.

Work with colleagues within ICB, Provider Trusts, Provider Collaborative and Local Authorities to support individualised commissioning when required.

To actively engage and work with colleagues and partners in both Place and across SWL to support project delivery.
To work in partnership with colleagues across South West London or Pan London to ensure alignment with other programmes creating joint working to benefit of the whole of SWL.

Leading the additional commissioning delegated functions from SWL ICB

To review and manage urgent admission to a mental health hospital out of area (acting on behalf of the ICB where Low Value Activity applies)
To review statutory provision requests in relation to MH needs for young people aged 18-25 under Section G of Education, Health and Care Plans (EHCP).
To support the ICB in the application for Court of protection or Deprivation of Liberty Safeguards (DOLS)
To provide oversight/review for discharge planning
