W e are recruiting a Discharge Coordinator to work as part of our multi-disciplinary team supporting the timely transition of patients with complex neurological disability into the community following a period of rehabilitation.
The Regional Hyper-acute Rehabilitation Unit (RHRU) is a 24 bed in-patient specialist neuro-rehabilitation service.
The RHRU is part of a specialist rehabilitation network and acts as a central focus for research and training of all professions in rehabilitation. The unit offers intensive multi-disciplinary neuro-rehabilitation to adults with severe and complex cognitive, communicative, physical and psychosocial difficulties. This may be due to traumatic brain injury, stroke or other neurological conditions.

The post offers an exciting opportunity for an enthusiastic and motivated individual to demonstrate your excellent interpersonal and communication skills. You will work closely with the specialist social worker and key workers and will be expected to liaise with a wide range of external agencies across the South East of England.
You will be able to use your initiative and have excellent organisational skills and prioritise a diverse workload under pressure.

Liaise with social services, statutory health, voluntary services, community teams and GPs/district nurses to ensure a smooth and coordinated discharge.
Help to co-ordinate the patient’s ongoing care and rehabilitation needs on discharge and assist in the development of a bespoke discharge plan for patients.
In conjunction with the keyworker and treating team, communicate with the patient and family regarding their choices and options.
Work co-operatively with all disciplines within the unit ensuring good inter-disciplinary team work to meet the discharge planning needs of patients with severe complex neuro disability, and their families.
To feed back issues that may arise with discharge planning that may impact on timely discharges and affect Delayed Tranfers of Care (DToC’s).
Provide a situation report on up coming discharges at the weekly referrals/admissions meeting.
Maintain patient’s clinical records in accordance with the agreed RHRU and Trust record keeping system and Documentation Policy, ensuring that all relevant information and communications with patients, family and relevant agencies are recorded.
Maintain a regular presence on the ward, attending relevant MDT meetings such as goal setting and training.
Coordinate implementation of the discharge schedule ensuring all the necessary arrangements are put in place to optimise discharge.
In liaison with the MDT, facilitate care training for family and friends on the unit to support the patient on discharge.
Chase referrals and test results if required.
Develop own knowledge and skills, and provide information, support and act as a point of reference for newer staff.
Manage patient discharges through booking transport, and problem solve any issues as they arise. This includes liaising with the Senior Pharmacist, and Dietitian.
