Job Overview

Minster Home Care are seeking a Care Coordinator to join our team to help facilitate the safe quality and secure delivery of care services.

A Care Coordinator is responsible for ensuring that clients receive comprehensive, well-managed care by coordinating/rostering the companies clients and staff accordingly. The role involves liaising between clients, employees and other relevant parties, so good communication and organisation skills are a must.

Job Summary:

The main role of a Care Coordinator is to schedule/roster clients and staff rota, in advance, to ensure smooth delivery of home care services.

Key Responsibilities:

Care Coordination and Rostering: Daily coordinating and rostering care runs by collaborating with clients and their families
Communication: Facilitate effective communication between clients, families, and other healthcare professionals..
Documentation: Maintain detailed records of clients interactions, care plans, and coordination activities.
Administration: Support office colleagues with phone calls, emails and general admin in line with business requirements

Training

Full training provided by IT team and senior management
Current care software has automatic scheduling to assist with coordination

Qualifications:

Experience: in healthcare, rota/scheduling or care coordination is required

Skills:

Strong organisational and time-management abilities.
Excellent communication and interpersonal skills.
Ability to work effectively within a multidisciplinary team.
Empathy and understanding of client and employees needs.
