About The Organisation

We proudly serve a population of 655,000 across rural and urban communities in both North and South Lanarkshire. NHS Lanarkshire is comprised of Acute Services (which currently provide hospital based services over 3 main sites), Corporate & Property & Support Services, North and South Lanarkshire Health and Social Care Partnerships which provide integrated primary healthcare and social care services to local communities and surrounding areas.

The Role

This is an exciting opportunity to work as part of a Primary Care Multi-disciplinary team. The CLWs will provide a person-centred approach in response to the needs and interests of GP practice patients supporting them to identify issues that affect their ability to live well. The CLWs will work collaboratively with patients supporting them through decision making processes to identify issues that they would like to address and help them to overcome barriers to improving these

The working pattern for this role is 36 hours over Monday -Friday core hours of 0800-1800 (to be confirmed)

Please note this is a fixed term post for until 30/09/2027.

Where a post only has temporary funding and an existing member of NHS Lanarkshire staff wishes to apply, this will be treated as a secondment. The employee MUST already have written agreement from their line manager to be released on a secondment before applying for the post

In this key role, you will:

To work in partnership with the Community Link Workers Co-ordinators to build new effective pathways and networks of support for patients and help patients to navigate existing complex pathways resulting in sustainable opportunities for patients to engage in meaningful activity.
Improve patients’ health and well-being by supporting them to access training, learning, social and support activities. This will involve empowering patients to develop new skills, confidence, relationships and contacts within their community.
To negotiate and support access for patients to community-based services and activities that will support them to live well. This may include sensitively challenging service providers, bringing direct benefits to patients.
To promote the Community Link Worker role within the Primary Care Teams to ensure appropriate referral and to provide feedback on outcomes of those referrals

What You'll Bring:

Educated to HNC level or above in a health or social care related subject or knowledge acquired through demonstrable experience
Experience of working with vulnerable adults.
Health improvement knowledge and practice
Highly effective interpersonal and communication skills in working with people on a 1:1 basis and within organisations.
Ability to work flexible hours as early evening work may be required at times.
Ability to work within the homes of patients on some occasions

A driving licence is required; this must be a full UK/EU/EEA licence.
