About the Role
This role is focused on responding to the needs and wishes of individuals, supporting them to lead full and meaningful lives through person-centred approaches. We are looking for someone who is compassionate, proactive and committed to helping people achieve the outcomes that matter most to them. You will take a personalised approach to care and support, promoting independence, choice, control and wellbeing.
In this role, you will need to work creatively and flexibly, building positive relationships with individuals, families, colleagues, professionals and wider community networks. You will be confident in supporting people to understand and manage risk, while ensuring their safety and dignity remain central to the support provided.
