National Care Service (NCS) is hiring a BAND 5 – Occupational Therapist (OT) within the Health & Social Care Workforce (HSCW), assigned to NCS (DCS) Reading.
As a BAND 5 – Occupational Therapist (OT) within our NCS (DCS) Reading Service Branch, you will play a crucial role in supporting individuals within Berkshire & Hampshire.
Your work will directly to contribute to the reduction of hospital readmissions by providing clinical, practical, emotional, and person-centred professional support and assistance to individuals at the comfort of their homes.
You will empower individuals to regain their independence and ensure their basic needs are met to a high-quality standard.
You will also be expected to maintain accurate records, uphold safeguarding practices, uphold confidentiality, and work collaboratively within our Health & Social Care Workforce (HSCW) and externally with local authorities and other key stakeholders.

This position encompasses both regulated and non-regulated activities, as defined under the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014, ensuring compliance with all Care Quality Commission (CQC) Fundamental Standards and the Care Act 2014
Asa Band 5 Occupational Therapist (OT) within the Health & Social Care Workforce (HSCW), assigned to NCS (DCS) Reading, you will manage a caseload of up to 50 Occupational Assessment (OA) monthly, completing person-centred and holistic evaluations to maximise independence and reduce risk within the individual’s home.
Each Occupational Assessment (OA) will consider functional ability, environmental risks, activities of daily living, mobility and safety. You will identify appropriate equipment, adaptations and Care and Support Needs.

The postholder must have:
- Valid Enhanced Disclosure Baring Service (DBS) with No Criminal Convictions.
- Valid Health and Care Professions Council registeration as an Occupational Therapist (OT).
- Right to Work in the United Kingdom (Proof Required Prior to Formal Interview).
