Full job description

We are a friendly, 3-site dispensing Practice in Buckinghamshire with surgeries in Wing, Whitchurch, and Winslow. The Practice has 6 GP Partners, together with salaried GPs and ANPs. They are supported by a full complement of Nurses, HCAs, a clinical pharmacist and provide care for 20,000 patients. We are a proactive and forward-thinking Practice dedicated to providing quality patient care. In total we are a team of more than 100 staff and we consider it important to maintain a supportive team ethos. We have a CQC rating of ‘good' across all metrics, thanks to outstanding administrative support.

We are looking for 2 part-time Clinical Coder & Scanners to work remotely.

Hours of Work to be discussed but to cover 08:00 to 18:00 Monday to Friday approx 22 hours per week.

Job Purpose

To provide an efficient and accurate document management and clinical coding service within the practice. The post holder will be responsible for scanning, indexing and processing incoming patient documentation, accurately coding clinical information using ANIMA, and ensuring electronic patient records are complete, upt o date and maintained to a high standard. The role supports patient safety, high-quality clinical care, QOF achievement, enhanced services and compliance with NHS information governance standards.

Key Responsibilities

Receive, prepare and scan all incoming paper correspondence into the clinical system.
Ensure scanned documents are clear, complete and attached to the correct patient record.
Accurately index documents using the appropriate document type and workflow.
Process electronic correspondence received via ANIMA or other NHS systems in accordance with practice procedures.
Identify duplicate or poor-quality scans and rectify where required.
Escalate documents that cannot be matched to a patient record.
Ensure all documents are processed within agreed practice timescales.
Maintain confidentiality when handling patient information and dispose of paper records securely in line with practice policy.
Read and interpret clinical correspondence, including hospital letters, clinic reports and discharge summaries.
Accurately code diagnoses, procedures, investigations, long-term conditions and relevant clinical information using SNOMED CT.
Update patient records promptly and accurately.
Maintain chronic disease registers by ensuring appropriate coding.
Support clinicians by ensuring patient records contain complete and relevant coded information.
Seek clarification from clinicians where documentation is unclear or incomplete.
Correct coding errors when identified.
Maintain accurate, complete and high-quality electronic patient records.
Contribute to improvements in data quality to support QOF, enhanced services and contractual reporting.
Participate in regular coding and data quality audits.
Assist with data cleansing exercises.
Identify missing or incorrect coding and rectify where appropriate.
Maintain strict confidentiality of all patient information.
Ensure secure handling, storage and disposal of confidential records
Comply with the UK General Data Protection Regulation (UK GDPR), Data Protection Act 2018 and NHS confidentiality guidance.
Complete all mandatory Information Governance and Cyber Security training.

Person Specification

Essential

Qualifications

GCSE (or equivalent) in English and Mathematics.
Good standard of general education.

Knowledge

Understanding of medical terminology.
Knowledge of SNOMED CT clinical coding.
Understanding of confidentiality and Information Governance.
Familiarity with GP clinical systems such as EMIS Web.

Experience

Previous experience of clinical coding within Primary Care or the NHS.
Experience scanning and processing clinical correspondence.
Experience working with confidential information.
Experience using document management systems such as Docman.

Skills

Excellent attention to detail.
High level of accuracy.
Good organisational skills.
Ability to prioritise workload effectively.
Excellent keyboard and IT skills.
Strong written and verbal communication skills.
Ability to work independently and as part of a team.
