Are you passionate about making a positive difference to people’s lives? Sheffield City Council is looking for motivated and compassionate Social Care Practitioners to join our Adult Social Care service and help us deliver high-quality, person-centred support to adults across the city.

Our Home First – First Contact Team provides a welcoming and responsive first point of contact for adults, families, carers and professionals seeking adult social care support in Sheffield.

The team offers a wide range of support, including information and advice, signposting to community services, preventative interventions, and the completion of person-centred assessments and support plans. Our focus is on understanding what matters to each person and supporting them to achieve outcomes that are meaningful to them.

You will work in a supportive and committed team that values collaboration, professional curiosity and respectful practice. We are looking for practitioners who can build positive relationships, recognise people’s strengths and networks, and support safe, proportionate and effective decision-making.

In this role, you will need to be organised, flexible and confident in managing your own workload, while also working closely with colleagues and partner agencies to provide a joined-up service for the people of Sheffield.

We are seeking applicants with recent experience in adult social care or a related field, good IT skills, and an understanding of key legislation and practice areas including the Care Act 2014, the Mental Capacity Act 2005 and safeguarding. Strong communication skills, excellent customer care and a commitment to collaborative working are essential.

If you are appointed to this role, your starting salary will be at the bottom of the grade. If you provide payslip evidence that your basic pay is greater than the bottom of the grade, we will consider starting you at the point of minimum advantage within the grade.
