We are looking for highly motivated Experienced Social Workers to join our journey and be part of a passionate service that is committed to improving the quality of lives of vulnerable adults across Sheffield. We are looking to recruit Experienced Social Workers in the following service areas:
1 x 2 year fixed position in Adult Future Options – Positive Outcome Review Team
2 x permanent positions in Maximising Independence Team
2 x permanent positions in The Neurodiversity Team
Adult Future Options – Positive Outcome Review Team

If the answer is yes, we have a 2 year fixed position available for an Experienced Social Worker.
As an Experienced Social Worker within the Positive Outcome Review Team, you will undertake strengths-based reviews with adults receiving care and support. You will work collaboratively with individuals, families, carers, providers and partner agencies to explore what matters most to people and how support can be delivered in a way that maximises their independence and improves their quality of life and well-being.
You will play a key role in promoting a strength-focused approach, recognising that people can continue to develop skills, confidence and independence throughout their lives when the right support is in place. Reviews will focus on identifying opportunities to build on strengths, reduce dependency, and empower people to achieve their aspirations.
The role also involves exploring how assistive technology and innovative solutions can enhance a person’s independence and enablement, whilst also supporting individuals to remain connected to their communities.
You will be a qualified and registered Social Worker with significant post-qualification experience in Adult Social Care. You will be confident in undertaking complex Care Act assessments, reviews, Mental Capacity Assessments and Best interest Decisions. You will have experience of balancing risk with personal autonomy and be able to professionally challenge the opinions of others. You will have a strong knowledge of key legislation and frameworks, including the Care Act 2014, The Mental Capacity Act 2005, the Mental Health Act and the Equality Act.

Someone who can work creatively with individuals, families, providers and professionals to achieve positive outcomes that promote independence. Maximising Independence Team - Experienced Social Worker
A new exciting opportunity for 2 experienced social workers to join our innovative team has arisen.
Would you like to work as part of a new multi-disciplinary team, working with young people who have complex and enduring mental health challenges, who are transitioning to adult social care?
The maximising independence team work with young people with enduring mental illness who are likely to need mental health support into adulthood.
We support people to become more independent by reassessing needs and supporting them move on from long-term provision via an MDT approach to (re)gaining abilities and skills, set enablement goals, and taking advantage of opportunities to grow and develop their lives.
Your role will be to work alongside other social workers, social care practitioners, occupational therapy and support workers to deliver key activities with targeted groups of people particularly young people moving form children's services into adulthood.
You will need to have a good understanding working with the Care Act 2014 and the Mental Capacity Act 2005, and the Mental Health Act and Look After Children Legislation.
Knowledge, skills and experience of supporting young people with complex mental health challenges to move from looking after children’s services to adult social care support would be beneficial in this role.

The Neurodiversity Team
Are you passionate about improving outcomes for neurodivergent adults and people with complex needs? We are seeking a motivated and experienced Social Worker to join our newly established Neurodiversity and Complex Needs Team.
This innovative multidisciplinary service has been created to provide high-quality assessment, intervention, and support planning for Neurodiversity adults with complex needs, ensuring people receive the right support at the right time. The team will play a key role in preventing crises, promoting independence, supporting transitions into adulthood, and reducing reliance on restrictive services.
As a Social Worker within the team, you will work directly with adults who require specialist assessment and intervention. You will undertake strengths-based assessments, develop person-centred support plans, and work collaboratively with individuals, families, carers, and partner agencies to achieve positive outcomes. You will manage a varied and complex caseload, applying your professional expertise to support people to live fulfilling and independent lives.
Working alongside Occupational Therapists and colleagues across Adult Social Care, Health, Housing, Education, Employment Services, and community organisations, you will play a key role in delivering coordinated and innovative support to neurodivergent adults and those with complex needs.
We are looking for someone who:
- Is a qualified Social Worker registered with Social Work England.
- Has experience working within Adult Social Care, including complex casework.
- Demonstrates excellent assessment, risk management, and care planning skills.
- Is committed to strengths-based, person-centred practice.
- Has experience working with Neurodiversity adults with complex needs.
- Can develop effective working relationships with individuals, families, carers, and partner agencies.
- Is passionate about improving outcomes, promoting independence, and supporting individuals to achieve their goals.

If you are appointed to this role, your starting salary will be at the bottom of the grade. If you provide payslip evidence that your basic pay is greater than the bottom of the grade, we will consider starting you at the point of minimum advantage within the grade.
