Care Co-ordinator (Non-Clinical) and Practice Administrator

Trowbridge Primary Care Network and Lovemead Group Practice, Roundstone Surgery

Hours: 37.5 hours per week, Monday to Friday, 8.30am to 5.00pm

Contract: Permanent

About Us

Trowbridge Primary Care Network (PCN) brings together local GP practices to deliver high-quality, joined-up care for our community. We are a friendly, supportive team working collaboratively across practices, care homes and community services to improve outcomes for our patients.

We are seeking a motivated and well-organised individual to join our team in a dual-role position, combining Non-Clinical Care Co-ordinator responsibilities for Trowbridge PCN with Practice Administrator duties at Roundstone Surgery.

Based at Roundstone Surgery, the successful candidate will work across both organisations, supporting coordinated care for care home residents, patients with learning disabilities and carers, whilst also providing key administrative support to the practice, including patient recalls, safeguarding administration and general practice administration.

About the Role

This is a varied and rewarding role which combines care coordination across the PCN with administrative support within general practice.

The successful candidate will support our local nursing homes, learning disability (LD) homes and carers, helping to ensure effective communication, organisation and continuity of care. In addition, they will work alongside the Practice Patient Services Team to support patient recalls, safeguarding processes and day-to-day administration.

Care Co-ordinator Responsibilities

Liaise with nursing homes and learning disability homes to arrange and support routine visits and reviews.
Organise clinic and ward round schedules for visiting GPs and clinical staff.
Coordinate annual learning disability health checks and support timely completion of assessments.
Support vaccination programme planning and scheduling for care home and learning disability home residents.
Maintain accurate records and ensure information is shared appropriately across the multidisciplinary team.
Provide administrative support for PCN care home and learning disability services.

Practice Administrator Responsibilities

Arrange recalls for patients with long-term conditions, ensuring reviews are completed in a timely manner.
Provide administrative support for safeguarding processes and vulnerable adults.
Support clinicians with administrative tasks relating to patient care and monitoring.
Work alongside the Patient Services Team providing general administrative support for our clinical team as required.
Maintain accurate patient records and ensure all information is processed confidentially and in accordance with GDPR and practice policies.
Act as the Carers Lead, supporting carers by ensuring they are identified, signposted to appropriate services and included in care planning where appropriate.

About You

Highly organised with excellent attention to detail.
Able to manage competing priorities and work effectively across multiple areas of responsibility.
Comfortable working independently as well as part of a wider multidisciplinary team.
A confident communicator, able to liaise professionally with care home staff, clinicians, carers, patients and their families.
Compassionate, professional and committed to delivering excellent patient support.

Essential Skills and Experience

Proven administrative experience in a busy working environment.
Excellent organisational and time-management skills.
Strong communication and interpersonal skills.
Proficiency in Microsoft Office applications, including Word, Excel and Outlook.
Ability to handle sensitive and confidential information appropriately.
Ability to prioritise workload and meet deadlines.

Desirable Skills and Experience

Previous experience working in primary care, healthcare or health and social care settings.
Experience of patient recalls, care coordination or safeguarding administration.
Knowledge of GP clinical systems.
