Playing an integral role in the health service, the main purpose of the role of a Care & Support Worker on the Hospital Discharge Team is facilitate the timely discharge of patients from hospital to prevent unnecessary delays and bed-blocking.
The key responsibilities are -
- To support and vulnerable persons to be discharged safely from hospital back to the home environment with appropriate care and support.
- This service is based upon a four week service delivery at which point individuals will be assessed by a Social Worker to formulate a long term plan where required.
- The individual may either then enter the main care stream or be re-assessed for differing needs.
The HDS Care & Support Worker must be aware of the needs and requirements of the person receiving services and as such you must communicate effectively not only with the individual but also with others such as family, friends and other healthcare professionals.
As an HDS Care & Support Worker you will always be briefed before undertaking any assignment with the specific tasks involved but in every case it is incumbent upon you as the HDS Care and Support Worker to carry out the work in a safe and professional way and to adhere to the specific instructions as well as the policies and procedures of the company.
It should be clearly understood that the role requires a great deal, sometimes both in physical and emotional ways, and also carries a weight of responsibility equally it can be very emotionally rewarding.
A thorough training programme is mandatory for every individual who joins Nationwide Care. This constitutes both initial and ongoing training and is a requirement of all staff. Every member of staff will be required to apply for, and achieve, a satisfactory Disclosure Certificate (DBS) and successfully complete initial training.

MAIN DUTIES AND KEY RESPONSIBILITIES
- To represent the Company in a professional manner at all times
- To deliver a positive first impression, remember you may be the only person that the service users sees all day.
- You must be prepared to log in and out of all calls using the Care Hub app in order that we can ensure the safety of you and the vulnerable persons we support.
- To maintain Confidentiality at all times
- Reliability is key
- Professionally liaise with Family, Next of Kin, GP’s, Social Workers, Local Authority Commissioners, Care Quality Commission and other Health Professionals at all times and as situations may require
- To act in a calm and professional manner when responding to emergencies or encountering challenging situations
- To seek help, assistance or support as and when you feel it is needed to assist you in supporting vulnerable persons or stakeholders
- To take responsibility to maintain professional knowledge and competences
- To apply good practice and report any/all suspicion of abuse of any kind to your seniors
- To undertake any additional duties appropriate to the scope of the position as maybe required from time to time
