We are recruiting for the the role of the Care Coordinator, to promote compassionate care for all our clients, making sure to utilise the care resources that are available, but also working closely with the recruitment team, with any further care resources required.
To maximise and align the clients’ requirements and needs within the defined quality, compliance, and safety standards.
You are a dynamic Care Coordinator with solid planning, scheduling and problem-solving experience with a flexible, enthusiastic and can-do attitude.
Key Duties will Include
- Allocating CareAngels with the right skills and experience to deliver the best quality care and support to each of our clients.
- Organise CareAngels rotas to minimise travel between clients.
- Respond efficiently to day-to-day changes in clients care needs and make changes where required.
- Monitor all relevant aspects of the care management system.
- Ensure that all client visits are allocated and reflect clients service orders/
- Ensure all re-assessments are completed in a timely manner and records are updated accordingly.
- Establish strong working relationships with all key stakeholders, both internally and externally.
- Undertake on call duties as per the office colleague rota.
Our Angels work with vulnerable adults in their local community giving support to help our clients live independently for as long as possible.
