If the answer is yes, we are seeking an exceptional individual to join our expanding “Trusted Assessment” team.

The appointed role holder will be responsible for the assessment and review of care and support services delivered to Service Users within our day time and night time services, and within extra care sites, joining a team of Trusted Assessors already in post.

Responsibilities:
- Carry out timely annual reviews, six-weekly reviews, and reviews following changes in circumstance, identifying risks and ensuring appropriate, proportionate risk management strategies are in place, including falls prevention and response.
- Monitor the safe delivery of care through observational visits, taking immediate action where practice does not meet required standards.
- Flex and adapt packages of care in response to changing needs, ensuring decisions are evidence-based, justified, and made collaboratively with Provider Led Review Teams and health partners.
- Provide professional advice and guidance to frontline staff and coordinators to support effective care planning and delivery.
- Respond to, report, investigate, and manage safeguarding concerns, quality issues, and incidents in line with organisational policy, statutory guidance, and KPI requirements, ensuring relevant partners are informed and involved.
- Ensure accurate, up-to-date risk assessments and support plans are in place and shared appropriately with all relevant agencies.
- Monitor and promote health and safety in the workplace and in the community, escalating concerns where required.
- Audit Electronic Call Monitoring (ECM) data to identify inappropriate, unsuitable, or over-commissioned care, ensuring alignment with Trusted Assessor guidance.
- Maintain accurate, clear, and timely records using organisational ICT systems to support continuity of care and decision-making.
- Provide clear and accessible information about services, expectations, and how to raise concerns or complaints.
- Promote a person-centred culture across teams, ensuring care delivery reflects compassion, respect, and empathy at all times.
- Support the team in emergency care cover, if deemed necessary as part of an exigency of the service.
Requirements:
- Hold a minimum of 6 months experience in a care setting either as a care coordinator, care assessor or care supervisor / manager with experience of assessment and review.
- The holder of a Level 2 QCF in Health and Social Care (minimum) or have achieved or be working towards a Level 3 QCF or above (desirable).
- Be proficient in the use of I.T systems.
- Holder of a UK Driving Licence with access to a vehicle.
- Strong communication and interpersonal skills
- Ability to work collaboratively in a multidisciplinary team.
Terms:
- Contract: Permanent - Full Time
- Hours: 40hrs
- Working Pattern: 4 Days per week
- Every Wednesday, Thursday, Friday, Saturday (days off Sunday Monday, Tuesday)
- Shift Times: 7am – 5pm core
- Role Commencement: ASAP
