Job Summary:
The Team Leader will oversee the daily operations of a small supported living service in Sittingbourne/ Gravesend for individuals with mental health conditions. This role involves supervising staff, ensuring the delivery of high-quality care, administering medication, and leading shifts to provide consistent and compassionate support for residents. The Team Leader will play a pivotal role in promoting independence, safety, and well-being in a supportive and inclusive environment.
Key Responsibilities:
Leadership & Team Supervision:
- Lead and supervise a small team of support workers, providing direction and guidance to ensure high standards of care.
- Oversee the effective running of shifts, ensuring staff are appropriately assigned and supported.
- Promote a positive, collaborative team culture built on our values of openness, inclusiveness, quality, and compassion.
- Mentor and coach support workers to enhance their skills and development.
- Conduct regular team meetings, provide performance feedback, and ensure staff are adhering to policies and procedures.
- Support staff with de-escalation techniques and provide guidance on handling complex situations.
Resident Care & Support:
- Provide direct support to residents with mental health conditions, including assisting with daily living tasks, medication administration, and personal care.
- Conduct and document care and risk assessments, developing care plans tailored to the assessed needs and choices of individual residents.
- Ensure that identified risks are appropriately monitored, with outcomes recorded and acted upon (e.g., blood pressure,
- Administer medication in accordance with established procedures.
- Ensure residents are encouraged and supported to remain as independent and active as possible, promoting the philosophy of activity-based care.
- Promote the safety and comfort of all residents, ensuring a happy, homely atmosphere.
- Liaise with GPs and other health professionals to ensure residents receive medical and clinical support in a timely manner.
- Support residents in building independent living skills, including budgeting, meal planning, personal care, and social skills.
- Ensure personal care is provided respectfully and in line with individual choices.
- Ensure compliance with health and safety regulations, including the management of infection control and food hygiene.
Health & Safety & Compliance:
- Ensure that care delivery adheres to CQC (Care Quality Commission) regulations and internal policies.
- Support the completion of risk assessments for each resident to ensure their safety and well-being.
- Report all accidents and incidents promptly in accordance with Star Care Lodge Policy, ensuring accident/incident reports are clear and concise and include remedial action or learning.
- Maintain a safe environment, ensuring that all health and safety standards are met.
- Be knowledgeable about Safeguarding Vulnerable Adults Policies and Procedures and report any untoward events in accordance with Star Care Lodge and Local Authority requirements.
- Conduct Mental Capacity assessments and review care plans every six months or as needed.
- Ensure that care plans are updated, and care reviews are conducted every six months and monitored monthly with key workers to ensure they are up to date.
Communication & Administration:
- Act as the point of contact for families, external agencies, and healthcare professionals regarding residents' care.
- Maintain accurate, confidential records, including daily diaries, incident/accident reports, and medication administration records.
- Support the administration of the service, including updating care plans and reviewing care needs as appropriate.
- Ensure the clear and consistent communication between team members during handovers, promoting transparency and teamwork.
- Facilitate service reviews and assist in care plan updates in consultation with residents and other relevant professionals.
Supervision & Quality Assurance:
- Formally monitor the care delivery on duty to ensure that all residents' needs are being met, ensuring that accurate records are kept.
- Participate in quality assurance and auditing processes, assisting with the formal evaluation of practices, including undertaking observational supervisions.
- Supervise care staff on a day-to-day basis and through formal supervision processes to ensure that they are providing high-quality care.
- Liaise with GPs and other health professionals to ensure that residents receive the medical and clinical support they require.
- Demand high-quality care delivery focused on residents' individually assessed needs and choices.
Professional Development:
- Support and encourage staff in their personal and professional development, including facilitating training and development opportunities.
- Take on an active role in the recruitment and induction of new staff members.
- Maintain an up-to-date knowledge of best practices in mental health care and supported living services.
Qualifications & Experience:
Essential:
- Level 4 or 5 Diploma in Health & Social Care (or equivalent).
- Proven experience in leading a team or managing a mental health or supported living service.
- Extensive experience of working in mental health and supported living settings.
- Experience in administering medication and managing care plans.
- Strong knowledge of CQC regulations, safeguarding procedures, and health and safety protocols.
- Excellent communication and leadership skills.
- Ability to work 12-hour shifts (8 AM – 8 PM), including weekends and bank holidays.
Desirable:
- Full UK driving license.
- Training in PMVA (Prevention and Management of Violence and Aggression) or MAPA techniques.
