At Plymouth Care is looking for a dedicated and organised Care Coordinator to join our team. This role is essential in ensuring the smooth delivery of high-quality care services to our clients in the Plymouth area. You will be responsible for coordinating care staff, managing schedules, and supporting both clients and carers to ensure safe and effective care delivery alongside our care supervisors and the registered manager.Key Responsibilities
- Coordinate and schedule care staff to ensure all care visits are covered efficiently
- Maintain accurate care records and update care plans where required, Supporting in care plan reviews and changes
- Monitor staff availability, sickness, and holiday requests
- Ensure services are delivered in line with company policies and regulatory standards
- Participate in the on-call rota to provide support outside of normal working hours
- Build positive relationships with clients, families, and healthcare professionals
Requirements
- Previous experience in a care coordination or similar role (preferred)
- Experience working within health or social care
- Strong organisational and communication skills
- Ability to work in a fast-paced environment and manage multiple tasks
- Good IT skills and experience using care management systems
- A compassionate and professional approach to care delivery
- Full UK driving licence (preferred)
